5. Install three -stage decontamination units, consisting of a clean room, shower and
equipment room, as appropriate, for each regulated asbestos removal work area.
Install sufficient HEPA-filtered negative air pressure machines, to change out the
air in the work area, and vent the exhaust outside the building.
6. Remove and dispose of the following types and quantities of existing asbestos -

7. After removal is completed, the work areas will be HEPA vacuumed and/or wet
wiped as appropriate and encapsulated to lockdown any remaining fibers.
8. Perform personal exposure monitoring of a minimum of 25% of the designated workforce
for compliance with OSHA 29 CFR 1926.1101.
9. All designated bags of asbestos & lead waste will be stored in lockable dumpsters on -site
and thentransported to a State of Florida licensed "Asbestos Landfill" facility.
L0. Provide a copy of all applicable "Post -Job" submittals including, but not limited to the
following; copies of LE's Daily Work logs, Exposure Logs, OSHA personal air
monitoring results, and a copy of the applicable asbestos waste manifest/waste shipment
receipt forms (WSRs).
Work by Others:
1. Removal and disposal of all non -asbestos -containing contents, furniture and
construction debris and power disconnection.
2, Provide temporary restrooms, water and electric service during the entirety of all
asbestos removal operations.
3. Supply an independent testing laboratory and State of Florida licensed Asbestos
Consultant to perform a final visual observation/inspection and/or final clearance
air sampling and analysis, with acceptable levels being less than or equal to 0,01
fibers/cc, as measured by Phase Contrast Microscopy,
Safety 444 (Manly Control Program:
1, Lang Environmental has an active safety program with a dedicated safety manager who
wilt monitor the project and conduct weekly safety meetings, as appropriate.
2. Personnel air monitoring analysis will be performed by an independent testing lab.
3. All personnel working on this project will have pre -employment physicals, and have the
medical examinations available for your review, in accordance with OSHA requirements.

Cross Environmental Services, Inc. (CES) is pleased to provide a quotation for the above referenced
project. The general scope of services associated with our quotation includes all labor, material,
equipment, supervision, and disposal associated with the asbestos removal and as defined per the
Southern Earth Sciences asbestos survey dtd. August 16, 2019.
Scope of Services:
Our specific scope of services includes the following items:
a Filing of required regulatory notices, to Florida Department of Environmental Protection (DEP), if
required.
a Asbestos certified personnel.
O Asbestos removal as defined per the Southern Earth Sciences asbestos survey cltd. August 16,
2019.
• Proper disposal of all ACM and C&D debris removed by CES.
1 Estimated 10 working days to complete.

• Equipment and labor.
Price is based on a single mobilization.
