Pattison Professional Counseling and Mediation Center (PPCC) submits this contract to provide an
Employee Assistance Program (EAP) For the City of Crestview including the number of counseling
sessions, additional services, and conditions specified below, and will comply with Required Assurances
services will begin _October _1 __, 2019 and will extend through September 30, 2020. A fee of $60 will
be charged for each counseling session.
Required Assurances:
1.Maintain referral and treatment records, ensuring confidentiality to EAP clients.
2.Submit a monthly report/invoice to human resource division of the City of Crestview in
statistical format pertaining to the number of EAP sessions utilized.
3.Provide each EAP client services equivalent to private clients (i.e. scheduling appointments,
level of expertise, emergency situations, etc.)
4.Provide the designated number of EAP sessions established in agreement to eligible City of
Crestview employees and/or their immediate family members -matching counselor expertise
and type of services with the client's needs.
5.Make appropriate referral for EAP clients that require inpatient services considering economic
resources available to the client and provide follow-up services.
6.Comply with established written polici€s and EAP procedures as provided by the City of
Crestview.

Number of counseling sessions including initial interview with the EAP counselor: 4
Eligibility: all City of Crestview employees or their family members can utilize up to four EAP sessions per
calendar year.
Rate/schedule for visits beyond EAP:
Pattison Professional Counseling and Mediation Center has clinicians that are preferred providers with
the insurance company provided by the City of Crestview. Clients wanting to continue counseling can
utilize their health insurance to pay for continued services. The cost of the client is minimal as we have
preferred providers that are in the insurance network the City of Crestview utilizes. However, if a client
does not have insurance, he/she can receive counseling based on our sliding scale fee. The client discusses
his/her financial situation with the clinician and a fee -for -service arrangement is determined. Therefore,
counseling is affordable to all clients who wish to continue counseling
Each client calling to access their EAP benefits are screened to determine what method of continued care
will best meet his/her needs. The client is then placed with an EAP counselor who has the expertise to
address their issues and who can provide services beyond his/her EAP based on a sliding scale or their
insurance carrier. If the client wishes to continue treatment beyond his/her
EAP sessions they will be able to continue with the same counselor in the most cost-effective manner.

Additional services to be provided:
In addition to the counseling services provided for your employees, PPCC will provide up to (2) one -hour
seminars/workshops for no additional cost to the City of Crestview on important topics such as time
management, stress management, anger management, communication skills, conflict resolution, sexual
harassment in the workplace, and drug/alcohol awareness. The seminars/workshops can be scheduled
any time management feels the need exists. Additional seminars provided beyond the first two sessions
will be provided at the rate of $100 per -hour.
Furthermore, as an employer/supervisor the agency can ask an employee to utilize their EAP benefits to
obtain an evaluation due to ongoing work -related issues. If the employee agrees to address and resolve
the issue utilizing their EAP benefits, they will be asked to sign a release at PPCC to allow communication
to occur between the therapist and the supervisor. If the employee agrees, ongoing communication will
continue until the issue is resolved.
The City of Crestview can ask for a fit for duty evaluation utilizing the employee's EAP visits. The therapist
will perform a mental status evaluation to assist in a fit for duty determination. The therapist may suggest
an additional evaluation from a psychologist for a more in-depth determination if needed.
Research indicates that employee assistance programs are very beneficial in resolving work -related issues
and increasing productivity in employees. A supervisor who would like to address a personnel issue in a
productive and positive manner can utilize the EAP to assist them in this process. Employees can also
receive great benefits from the EAP by resolving personal issues that cause disruptions in the work
environment.
Confidentiality is a very important part of an EAP. All individuals seeking counseling services according to
Florida law have the right to confidentiality. Therefore, records of all City of Crestview employees and
their family members who access their EAP benefits are kept confidential. The human resource
department will only have access to the number of services provided by PPCC to review utilization.
Employees utilizing their EAP services will be assigned a number. The monthly statement submitted to the
City of Crestview will include the employees assigned number and the amount of sessions utilized by the
employee with their family members.
Special characteristics/advantages:
Each City of Crestview employee can receive four counseling sessions from a Licensed Mental Health
Counselor or a Licensed Social Worker. Counseling services may include individual, family, or group
sessions depending on the client's needs.
We have two modern facilities located in both North and South Okaloosa County making our services
conveniently available to all Oka loosa County residents. Our offices are open Monday through Friday from
8 AM to 8 PM with weekend appointments available upon request.

PPCC will provide timely counseling services to all employees. In emergency situations PPCC can provide
counseling services within 24 hours of the client calling. If the client calls Monday through Friday PPCC
can provide counseling services, the same day the client calls to access their EAP benefits. Furthermore,
all therapists carry cell phones and are ava.ilable to the client for emergency services.
PPCC will provide Critical Incident Debriefing (CIDS) treatment upon request at a rate of $120 an hour.
Licensed therapists will provide the treatment in group or individual sessions at a location determined by
the City of Crestview. CIDS are utilized for high stress events that can emotionally traumatize an
employee.
PPCC provides a wide range of services. We have clinicians that specialize in the treatment of substance
abuse, domestic violence, parenting, and anger management. Group or individual counseling sessions can
be offered in each of these specialized areas.
We also have clinicians who specialize in the treatment of depression, anxiety, bipolar disorder,
obsessive-compulsive disorder, phobias, childhood developmental disorders, ADD/ADHD,
marriage/family dynamics, parenting, stepfamily dynamics, trauma/abuse issues, Christian counseling,
and addictions.
As a referral source PPCC has a licensed psychologist who can provide psychological testing as needed.
