The scope of the project builds upon the identified projects within the City of Crestview Community Redevelopment Agency Mobility Plan (Mobility Plan). Our integrated team of engineers, urban designers, and analysts will bring our combined experience to successfully deliver this project. Deliverables: • Conceptual design documents with revised Engineer's Opinion of Probable Cost (EOPC) • Construction documents, plans, specification, and (EOPC). Task 1.1- Project Coordination — Matrix will ensure all agency stakeholders identified at the onset of the project are engaged throughout the project. It is assumed the following agencies will be impacted: • City of Crestview (Crestview) (8 meetings assumed) • Gulf Power (1 meeting assumed) • Other utility providers (2 meetings assumed) • Miscellaneous communication companies (2 meetings assumed) Task 1.2 - Project Management — Matrix will serve as the design project manager for this scope. There are several stakeholders that will be involved with the project, and we will actively manage through a series of meetings and other forms of communication. Assumed meetings and information include: • Project Kick-off meeting (1 meeting) —we will schedule and conduct a project kick-off meeting with Crestview to discuss and confirm the vision and goals of the project as well as discuss project schedule and next steps. • Progress meetings (26 meetings) —we will schedule progress meetings on a bi-weekly basis. These meetings will report on the progress of the project including project activities completed to date, schedule progress, follow-up on action items from previous meetings and discussion of new issues that have been uncovered. A set of minutes and action items will be distributed to all attendees. • Monthly progress reports/invoices — we will provide a progress report each month that is attached to our monthly invoice. The progress report will identify tasks that were worked on during the previous month, tasks that are anticipated to be worked on the following month, and identify any issues that may impact the scope, schedule or budget of the project. • Monthly schedule updates — we will update the project schedule monthly and be able to show progress against the agreed -upon baseline schedule. Page 2 for Matrix Task 1.3 - Public Outreach — Our team will support the City with efforts related to public outreach and communications for the project. We will prepare materials for public meetings and meetings with council. We have assumed that City staff will conduct public meetings and provide feedback to the design team. In addition, we have also assumed that City staff will provide updates with council and provide feedback to the design team. Construction Documents Project Scope Definition The Matrix team has developed scope for the Main Street project based on individual projects identified within the Mobility Plan, the proposed grouping of projects for this scope of work is listed below: Group 1— Projects north of Railroad Avenue, Main Street Focused Group 1A (Main Street US 90 — Railroad Avenue) Mobility Study Project ID Project / Description 1 Cedar Ave at US 90 Intersection Improvements — Design to modify the intersection of Cedar Avenue with US 90 2 & 3 Main Street from US 90 to Industrial D Roadway and Streetscape Improvements -Design of 5 8 N/A Main Street roadway and streetscape improvements from US 90 to Industrial Drive. Pine Avenue Roadway and Streetscape Improvements — Design for roadway and streetscape improvements for Pine Avenue from Wilson Street to Main Street Beech Avenue Roadway and Streetscape Improvements — Design for roadway and streetscape improvements for Beech Avenue from Wilson Street to Ferdon Boulevard Oakdale Avenue Roadway and Streetscape Improvements — Design for roadway and streetscape improvements for Oakdale Avenue from Wilson Street to Ferdon Boulevard N/A Woodruff Avenue Roadway and Streetscape Improvements — Design for roadway and streetscape improvements for Woodruff Avenue from Wilson Street to Ferdon Boulevard Task 2 — Pre -Design Activities The Matrix team will complete a series of data collection activities upon receiving notice to proceed. These data collection activities will include the following: Task 2.1- Topographic Survey — The Matrix team will provide a topographic and right-of-way survey for the project limits. Survey will include location of all right of way boundaries, topographic features, drainage features, doorways and building access points, and located utilities. The survey team will work with a utility locator to determine below grade facilities. Page 3 Task 2.2 —Subsurface Utility Investigation — Matrix will follow the Subsurface Utility Investigation best practices to determine the location of underground utilities within the limits of the project. We assume potholes will be required. As part of our preliminary design we will determine locations of potholes and coordinate the locations with the City. Once the need for potholes are finalized, we will provide a task order for the potholing activities. Task 2.3 - Geotechnical investigation — The Matrix team will provide geotechnical investigation to determine the existing soil conditions to inform pavement design. Task 2.4 — Environmental investigation — Matrix will provide a Phase 1 Environmental Site Assessment. Once Phase 1 is complete, we will provide recommendations to the City for any additional study that may be needed. Task 3 - Construction Documents Construction documents will be prepared for the submittal to the City of Crestview for infrastructure improvements and submitted through the City's review process. Packages will be submitted to the City at 60%, 90% and 100% design. The following plans will be submitted: Task 3.1 - Roadway Plans— Matrix will prepare roadway construction documents (per City standards) based on the design criteria established in Task 3 and based on the Mobility Plan defined scope. Matrix will include sidewalk grading for the sidewalk and streetscape improvements for the project, all grading in the pedestrian way and access points into buildings will meet all Americans with Disabilities Act requirements, Title II. Task 3.2 - Utility Plans— Matrix will work with impacted utility companies to identify infrastructure that needs to be relocated to accommodate the new roadway alignments. We will prepare plans that show these impacts as well as potential areas where the utilities can be relocated. Communication between the project team and the impacted utility companies will be continuous throughout the project design. Matrix will also prepare a utility matrix that will show the utility, utility owner, contact information, facility, as well as additional data. This task does not include design by Matrix and only includes coordination and identifying potential conflicts for the respective utility company's design. Task 3.3 - Potable Water Plans - Matrix will provide water plans per City of Crestview standards and requirements for the project. We assume new facilities will not be required, and the design will focus on facilities that need to be relocated based on the roadway design. Task 3.4 - Drainage Plans - Matrix will provide stormwater plans per City standards and requirements for the project. Plan and profile will be provided for below grade facilities. Design for water quality will be provided within the plans including plans and details for the improvements. Matrix will also analyze the drainage improvements along Oakdale Avenue near the intersection of Main Street where there are areas of inundation / minor flooding. Our team will prepare recommendations for improvements to reduce inundation / flooding at the drainage system to the City. Task 3.5 - Erosion Control Plans 'Matrix will provide erosion control plans for the project. The plans will include erosion control measures that meet the requirements of the City as well as appropriate details. Matrix will rely on the Contractor to apply for and obtain any state permits that may be required. Page 4 Task 3.6 - Landscape, Streetscape & Urban Design —The Matrix landscape team will prepare detailed design for the landscaping and streetscape for Main Street from the US 90 to Railroad Avenue This will include full streetscape design for sidewalk paving and streetscape elements such as furnishings, lighting, etc.; coordination with drainage elements within the streetscape; selecting types of plant materials and locations of plant materials that will be low maintenance; and, a designed streetscape that promotes the City's vision of the street. These plans will be included in overall plan submittal and submitted to the City for review. Task 3.7 - Irrigation Design —Irrigation plans for the project within the existing and proposed right-of- way will be provided for review and approval. Our team will verify the existing site conditions including the existing irrigation system and water tap connections as well as study the need for connections to future facilities to right size the design of the irrigation system. Task 3.8 - Construction Phasing and Traffic Control Design — Matrix will prepare construction phasing plans and high-level traffic control plan sheets that implement the proposed construction phasing. Task 3.9 - Specifications — Matrix will prepare technical specifications along with measurement and payment specifications for all bid items required to construct the project as part of the final construction document set. These will be prepared as revisions to FDOT specifications. Landscape specifications will be written as Special Provisions to FDOT specifications. Task 3.10 - Cost estimate — Matrix will prepare an opinion of probable construction cost based on our final design plans and specifications. Upon review of the opinion of probable construction cost, the project team can determine if any scope items should be added or removed or if any bid alternatives should be created. Task 3.11 - Submit Review Design Packages — The project team will prepare a design package at the 60%, 90% and 100% design levels consisting of the following items: • Drainage report • Roadway plan and profile design • Storm drain plan and profile design • Utility Plan Matrix and utilities to be potholed • Streetscape / Landscape plan • Engineers Opinion of Probable Cost • Construction phasing These deliverables will be submitted to the City for review and approval, we assume any referrals will be made by the City. Task 3.12 - Design Review Meetings (3) — Matrix will schedule and conduct design review meetings where all impacted stakeholders can attend and review their comments on the design package. Matrix staff will take meeting notes to document the comments received and will prepare a design comment matrix with responses for distribution. Stakeholders will have an opportunity to review the Matrix proposed responses so that a consensus can be reached on how to address the design comments. Task 4 - Engineering Reports Page 5 `iii Matrix In addition to the construction documents, reports are required by the City to gain approval. Reports include a drainage report, sanitary sewer report, and traffic impact study. Task 4.1— Drainage Plan and Report — Matrix will provide a drainage report for the limits of Group 1A defined above. The report will provide the necessary hydrologic and hydraulic calculations per City standards to project the amount of developed storm water runoff and to size the required storm sewer system to convey the developed runoffs. Water quality capture volume and required sizing calculations will also be provided in the drainage report. Task 5 - Final Plans Matrix will address any final comments that are provided from the review of the 100% design package and submit for approval. Comments will be addressed and coordinated with the review agencies, and final Construction Drawings (CD's) will be prepared and submitted to the City for approval. The CDs will contain civil plans necessary for the City of Crestview to bid and construct the improvements for the project. Task 5.1 — Final Plans: The Matrix team will prepare a final plan set for approval by the City, per the City's standards. Task 5.2 — Specifications: Our team will prepare final specifications for the project to be included in the bid documents. Specifications will include measurement and payment for bidding. Task 5.3 —Final Engineers estimate: Matrix will prepare a final opinion of probably construction cost based on our final design plans and specifications. Task 6 — Bidding Documents Matrix will prepare the construction documents for bidding through the City's process. It is our understanding that the following are required for bidding: • Advertisement for Bid. • Request for Bid. • Development of Evaluation Criteria we have assumed a Best Value Approach. • Evaluation of bids and recommendations of selection to City. • Preparation of Notice to Award. We have assumed that the City will provide all documents for standard City requirements and contracts. Exclusions and Assumptions The following services are excluded from this scope of work and task order: • Environmental Limited Site Investigation / Phase 2 are excluded from the scope. • All agency fees are excluded and shall be paid directly by the Owner • All work will be completed in AutoCAD and MS office format and as required by the review agencies. Page 6 Matrix • Infrastructure design not stated in the above text is excluded. • Supporting documentation for financing or regulatory packages that are not specifically referred to in the scope of services is excluded. • Construction management, construction inspections and construction support services are excluded and will be included in a future task order. • Improvements to utility infrastructure that is not identified in the scope above is excluded. • Construction Management and Inspection will be provided in a separate scope and is not included in this proposal and fee. Page 7 1r Matrix Recommended Future Studies The Mobility Plan identified several additional projects to improve the overall mobility for all modes. We suggest that the City continue to develop a comprehensive plan to implement and incorporate the mobility projects into the City's capital improvement plan. Our recommendations would include grouping of projects, as shown below, to create design and construction programs, developing conceptual design and defining or refining capital cost to implement the improvements.
