THIS ADDENDUM, (Addendum Number 1 to contract No. 83-2018, with Green 3,
LLC) is made and entered into this 2+� day of Nara, , 2022, by and between
• Richmond, Indiana, a municipal corporation acting through its Board of Public Works and Safety
with its office at 50 North 5th Street, Richmond, Indiana, (hereinafter referred to as "City"), and
SJCA P.C., 9102 North Meridian Street, Suite 200, Indianapolis, Indiana, 46260 (hereinafter
referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 83-2018 on May 22, 2018,
during which time Contractor was operating under the name of"Green 3, LLC"; and
WHEREAS, Contractor has recently gone through a merger with Green 3, LLC, and a
corresponding name change to SJCA P.C. with a change in address as reflected above
and as evidenced by correspondence attached to this Addendum as Exhibit A; and
WHEREAS, Parties wish to expressly agree that Contractor will operate under its new
name of"SJCA P.C." for purposes of finalizing payments due and payable to Contractor
under its former name.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein
contained, including the above stated recitals, City and Contractor hereby agree as follows:
1. The City and Contractor executed and entered into Contract No. 83-2018 dated
May 22, 2018.
2. Both Parties agree that Contract No. 83-2018 shall remain in full force and effect.
3. Both Parties agree that Parties shall continue to fulfill the obligations and duties
listed within Contract No. 83-2018.
4. Both parties acknowledge that Contractor has submitted or will submit a New
Vendor Tnformation form to the City Purchasing Department.
5. Both parties agree that with the exception of the name change and the address
change, Contract No. 83-2018 shall remain intact and unchanged.

6. Both parties agree that any person executing this Contract in a representative
capacity hereby warrants that he or she has been duly authorized by his or her
principal to execute this Contract.
IN WITNESS WHEREOF, this Addendum No. 1 to Contract No. 83-2018 is executed
and approved by the undersigned representatives of the City and Contractor this oh('day of
