THIS CONTRACT, made and entered this 91'/ day oft_ ., 2021 between the City
of Cre viewh r i st e e e Wafter called CITY and arfy?/t'. 1 Tr)tn hereinafter called
Athletics Official.
WITNESSETH:
WHEREAS, the CITY COUNCIL has directed Parks and Recreation Department to provide
youth sports for the CITY.
WHEREAS incumbent CITY personnel resources cannot provide the manpower required to
implement this program.
WHEREAS it is deemed prudent, wise, and beneficial for the CITY to utilize the services of local
residents that possess the capacity to function in a contractual work environment.
NOW, THEREFORE, THE PARTIES HERETO AGREE AS FOLLOWS:
ARTICLE I
The Athletics Official shall be responsible to provide services for the CITY to wit:
1. Be present at sports games
2. Ensure the rules are followed throughout the game
3. Be the responsible party for supervising the match
4. Any other related activities to officiating a sports match
ARTICLE II
1. Provide services based on a schedule determined by the Parks Supervisor
2. Accept the sum of $20.00 per hour, to be paid weekly PW
Accept the sum of $30.00 per hour, to be paid weekly TL/JV
Accept the sum of $35.00 per hour, to be paid weekly VAR
Accept the sum of $15.00 per hour, to be paid weekly CLOCK/BOOK
3. The CITY shall not withhold or match Federal withholding tax, Social Security, or
anything other amount requested; an IRS Form 1099 will be provided at the end of the
taxable year.
4. Receive direction and supervisor by the Parks Supervisor
5. Provision of this contract may be amended by either party subject to written notification
and agreement by the other party.
