TASK ORDER 2022-01
THIS TASK ORDER IS ISSUED PURSUANT TO THE STANDARD MASTER AGREEMENT FOR PROFESSIONAL SERVICES
DATED MAY 11, 2021 (AGREEMENT), FOR THE CITY OF CRESTVIEW, FLORIDA, WHICH IS INCORPORATED HEREIN BY
THIS REFERENCE, WITH RESPECT TO
Professional Engineering and Consulting Services for Design and Permitting of
Redstone Avenue Realignment and Widening from State Road (SR) 85 to west of
Brookmeade Drive.
Article A. Purpose
The City of Crestview (CLIENT) has seen significant population growth in recent years and that growth has led to an
increase in traffic which has created the need to improve access management along the main thoroughfares in the
City, specifically along Redstone Avenue, east of State Road (SR) 85 (S. Ferdon Boulevard) and west of Brookmeade
Drive. Redstone Avenue consists of commercial developments (primarily medical related uses) along this corridor.
There is also a commercial shopping center at the northeast comer of the signalized intersection of SR 85 and
Redstone Avenue. To the east of Brookmeade Drive there are over 900 new residential units that have been
constructed in just the last five years, with more units planned in the future. Riverside Elementary and Shoal River
Middle Schools also lie along this corridor and promote traffic along Redstone Avenue.
Existing issues that will be corrected or improved as part of this Task Order include straightening out the alignment
between SR 85 and Brookmeade Drive. We will also analyze the SR 85/Redstone Avenue signalized intersection
causing a bottleneck of traffic generated from the uses described above to see if a modification can be made to the
lanes and/or signal configuration to promote better traffic flow. The signal reconfiguration design services are described
below under Task 9 - Optional Services. There are also non-standard curb and gutters along the corridor that have
steep slopes directly adjacent to the travel lane and present a safety issue, especially for larger vehicles such as fire
trucks and ambulances. Lastly, there are no existing pedestrian features along this project corridor.
The eastern limit of this project will be coordinated with another design project which will reconfigure the intersection
of Brookmeade Drive and Redstone Avenue. The Brookmeade Drive intersection design is not a part of this proposal
except for coordination to ensure the designs mesh with one another.
Article B. Scope of Services
Introduction
The purpose of Task Order 2022-01 is to authorize Alday-Howell Engineering, Inc. (AHE) to proceed with the
professional engineering and consulting services for traffic analysis, design, permitting and bidding of the traffic
improvements from SR 85 to west of Brookmeade Drive. The construction of this project will utilize the existing right of
way between SR 85 and Brookmeade Drive but may also require right of way acquisition which will be evaluated as
part of this scope. See Figure 1 for the project location. As a part of this scope, AHE will design a straightened alignment
that provides additional queue length for the SR 85 turn lanes and additional lanes, as warranted. New curb and gutter

The scope of services provided with this task order is outlined in detail below.
Task 1— Data Collection and Traffic Analysis
This task will be performed by AHE subconsultant Panhandle Engineering and Constructions Services (PECS).
Task 1.1— Data Collection
This subtask will include collecting data related to turning movements counts (TMC) at the signalized intersection of
SR 85. These counts will be obtained during a period covering the A.M. and P.M. peak periods. PECS will also
collect relevant safety and collision data using Signal Four Analytics for the latest five-year period. PECs will also
coordinate with Okaloosa County Traffic Engineering for signal timing data.
Task 1.2 — Trip Generation
PECS will conduct a study of the committed and approved planned developments for the area and perform a trip
generation and distribution of traffic (based on exisitng patterns) to project background traffic conditions for the
project opening year.
Task 1.3 — Signalized Intersection Analysis
Using HCM software systems, PECS will determine exisitng and background operating conditions at the subject
intersection. Proposed modification scenarios will also be developed for AM and PM peak conditions and compared
against baseline conditions to determine any anticipated improvement in operations. Model conditions will also be used
to estimate the queue length at the intersection.
Task 2 — Site Investigations
Site -specific investigations will be required to obtain the field data needed to evaluate site characteristics, design
requirements, and potential impacts of realigning and constructing new roadways. The site -specific investigations,
which will be completed as part of this task, include a general assessment of site characteristics.

Task 2.1— Geotechnical Exploration
This subtask will be performed by Environmental and Geotechnical Specials, Inc (EGS) and includes mobilization to
the site and preparing a Geotechnical report. EGS will conduct lab tests to include Particle size analysis every 5 feet
(13 tests), Plastic limit and liquid limit every 20 feet (3 tests), and Organic test every 20 feet (3 tests). The field
investigation will include mobilization and the following borings:
• Roadway: Install 9-5 foot Hand Augers (45 feet)
• Pavement Survey: Install 4 Pavement Cores Coupled W/ 5 foot Hand Augers (20 feet / 4 Cores)
Report will include the following: Subsurface materials encountered, Measured groundwater and estimated "normal"
seasonal high groundwater, Geotechnical recommendations for roadway (over -excavation, subgrade preparation),
Evaluate pavement cores to determine asphalt thickness, layer composition, and base composition, and provide the
existing structure number, based on the FDOT visual identification model.
Task 3 — Conceptual (30%) Plan and Engineer's Opinion of Probable Construction Cost
The intended outcome of this task will be to obtain preliminary design approval from the CLIENT of the traffic
improvement project. A conceptual plan will be developed using the 2018 Manual of Uniform Minimum Standards for
Design, Construction and Maintenance for Streets and Highways (2018 Florida Greenbook), showing the geometry of
the improvements and the anticipated limits of construction. Under this task we will also determine if any work will be
required to modify the signal configuration at the SR 85/Redstone Avenue intersection. An Engineer's Opinion of
Probable Construction Cost (EOPCC) will be prepared in conjunction with this conceptual plan for the CLIENT's review
and comment. AHE will conduct a preliminary design review meeting to review comments and obtain any additional
questions that should be addressed in the final construction plans and specifications.
Under this Task, AHE will deliver the following items:
• Conceptual (30%) Plans for roadway realignment project. Two (2) hard copies and one (1) PDF copy will be
submitted.
• Engineer's Opinion of Probable Construction Cost
Task 4 — Conceptual Permit Application
The intended outcome of this task will be to obtain conceptual approval from FDOT and FDEP of the planned work by
the CLIENT. We anticipate that this project will require an individual ERP permit as well as State 404 Permit and UMAM
preparation. Upon acceptance by the state agencies, this conceptual approval will be used to develop a detailed design
for construction permitting. The team will coordinate with the regulatory agencies to attempt to obtain a reviewed
exemption or general ERP; however, if the regulatory agencies determine that an individual ERP is required the team
will prepare a supplemental amendment in the amount of the permit application fees.
Task 5 — Detailed Plans and Specifications
Upon completion of the Conceptual Approval phases, AHE will provide design services to develop construction plans
and detailed technical specifications for the realignment project utilizing the 2018 Florida Greenbook. AHE will prepare
the following submittals and transmit to the CLIENT for review and comment:
Subtask 5.1- 60% Plans Submittal
The 60% submittal package will provide detailed component drawings that include Roadway, Signing and Pavement
Markings, Drainage, Survey Control, Maintenance of Traffic, and a list of pay items to be used for the project.
Signalization and Structures plans will be included if the Traffic Study determines that lanes or signal pattern need to
be reconfigured.

Subtask 5.2 - Utility Coordination
Utility Coordination for this project will be conducted by AHE. This coordination will include field meetings with individual
utility owners, a utility design meeting, reviewing markups and work schedules, and conducting an in-house utility
constructability review.
Subtask 5.3 - 90% Plans Submittal
The 90% submittal package will provide detailed component drawings that include Roadway, Signing and Pavement
Markings, Drainage, Signalization (as needed), Survey Control, Maintenance of Traffic, and Summary of Quantities. A
detailed EOPCC and construction time estimate will also be provided at this phase as well as, draft front end and
detailed technical specifications.
Subtask 5.4 - Final (100%) Plans Submittal
AHE will compile comments from the 90% deliverable and finalize any remaining information into a Final Plans
Submittal package including detailed drawings and specifications for all components.
Under this Task, AHE will deliver the following items (two hard copies and one PDF copy will be submitted at each
phase):
• 60% Plans Submittal
• 90% Plans Submittal
• Final (100%) Plans Submittal
Task 6 — Construction Permits
The intended outcome of this task will be to obtain construction permits from applicable agencies such as FDOT, FDEP,
NWFWMD and USACE for the planned work by the CLIENT. This task will be conducted by AHE upon review and
approval of the 90% plans submittal.
Under this Task AHE will deliver all permits required for the construction of the realignment project.
Task 7 — Bid Services
• AHE will assist with bidding services for the construction of the traffic improvements project.
• CLIENT will advertise the project for bidding.
• CLIENT will provide electronic copies to of the bid documents to prospective bidders, equipment suppliers, or
other parties upon request.
• AHE will respond to requests for information and produce addenda to the bid documents to address questions
and changes discovered during the bid process.
• AHE will coordinate and attend a pre -bid meeting. AHE will prepare meeting minutes and distribute to plan
holders electronically as part of an addendum.
• AHE will attend the bid opening and tabulate the bid proposals, analyze the bids, and make a recommendation
to the CLIENT for award of the contract pending legal review.
• AHE will prepare conformed documents that will incorporate the changes made during the addenda. These
will be distributed to the CLIENT and selected Bidder for execution.

Under this Task, AHE will deliver to the CLIENT the following items:
• Up to three (3) addenda
• Pre -bid agenda and meeting minutes
• Bid Tabulation and recommendation of award
Upon selection of a bidder by the CLIENT, AHE will prepare five (5) half-size sets of the Conformed Contract
Documents for execution and use during construction. AHE will also provide an electronic copy to the selected
Bidder and CLIENT.
Task 8 — Services During Construction
AHE will make visits to the site to attend the preconstruction meeting and monthly construction progress meetings, as
well as additional visits at key milestones during construction. The Schedule of these visits is anticipated to be such
that it can maximize the purpose of the visit at key points during construction and on an "as needed" basis. Each site
visit will require approximately 4 hours, including field observations and report of findings. Anticipated site visits include:
• Preconstruction conference
• Six (6) monthly progress meetings
• Six (6) "as -needed" visits
• Twelve (12) bi-weekly site visits
• Substantial Completion and final punch list visit
• Final Completion visit
Final Inspection
AHE will review the work at the substantial completion and final completion stages. AHE will prepare a final punch list
of items requiring completion or correction. AHE will make recommendations to CLIENT regarding acceptance of the
work based on the results of the final inspection.
Task 9 — Optional Services
Optional Task 9.1— Geotechnical Mast Arm Investigation
This subtask includes mobilization to the site and preparing a Geotechnical report. EGS will conduct lab tests to include
Particle size analysis every 10 Feet (3 tests), Plastic limit and liquid limit ever 20 Feet (2 tests), and Organic test every
20 Feet (2 tests). The field investigation will include mobilization and the following borings:
• Mast Arm: Install 1-30 Ft. SPT Soil Boring (30 Feet)
Report will include the following: Subsurface materials encountered, Measured groundwater and estimated "normal"
seasonal high groundwater, Geotechnical recommendations for proposed mast arm foundation.
Optional Task 9.2 — Environmental Investigation
This subtask will include wetland delineation for the project area to be picked up by the survey crews. AHE will
subcontract with Environmental and Geotechnical Specialists, Inc. (EGS) to perform these field investigations for the
traffic project.
Optional Task 9.3 — Signalization and Structural Design for Mast Arm relocation
This task will be performed by AHE subconsultant Patel, Greene & Associates, LLC (PGA) if determined during the
conceptual phase that the signalization at the SR 85 intersection needs to be reconfigured. PGA will provide
signalization and structural design analysis and plan services to replace existing mast arms with new mast arms located
at the SR 85/Redstone Avenue intersection.

Optional Task 9.4 - Right of Way Mapping
The right-of-way requirements will be submitted during Task 3 above and shall identify, the existing right-of-way,
required right-of-way, temporary construction easements (TCEs), perpetual easements, intended license agreements
(LAs), and limits of construction. Maps and Legal Descriptions will be prepared for the properties to be acquired.
Assumptions
The following assumptions have been taken into consideration in the preparation of this Scope of Services and
compensation. These assumptions are based on the project scope, which is to deliver the CLIENT a completed set of
construction documents, contract documents, plans and specifications of sufficient detail to obtain a reasonably
accurate bid for services to construct the proposed improvements, and for the Contractor selected to be able to
construct the improvements and to assist the CLIENT with the monitoring of the construction of the project, and to
provide limited post -construction assistance.
Should deviations from these assumptions be required to deliver the services provided in this Scope of Services, the
scope of work and compensation shall be modified accordingly and approved by the CLIENT in writing prior to
executing the changes to the Scope of Services.
• The CLIENT will be responsible for providing survey data and utility designations within the exisitng right of
way of the proposed project limits and any areas that may need to be acquired in order to construct the project.
• The CLIENT will be responsible for the acquisition of land needed to complete this project. This includes
making offers and negotiations with property owners.
• The CLIENT will conduct public awareness campaigns to inform stakeholders of the project and its anticipated
construction timelines.
• The CLIENT will pay all permit fees for applications and subsequent reviews. Impacts are assumed to be
accomplished by the CLIENT's purchase of Mitigation Bank Credits (if required). Costs for mitigation credits
are to be paid for by the CLIENT.
• It is assumed that, aside from the Services During Construction listed in Task 8, the CLIENT will perform all
inspection of day-to-day construction activities.
Article C. Compensation Provisions
As compensation for providing the services described in Article B. Scope of Services in this task order, the CLIENT
shall pay AHE, in accordance with Section 5 of the Agreement, on a lump sum basis in the total amount of $120,350.
The total lump sum fee for Optional services is $76,210. These fees will be billed monthly by percent of completion.
The estimated breakdown of fees is shown in Table 1 below.

Work will begin upon execution of this task order. A preliminary milestone schedule is shown below. The total estimated
timeframe to deliver the final construction plans is 40 weeks from task order execution.
