TASK ORDER 2
This task order amendment is issued pursuit to PROFESSIONAL ENGINEER CONTINING
SERVICES CONTRACT dated May 3, 2021 between Matrix Design Groun. ("Matrix") and The
City of Crestview, Florida ("CLIENT") for services performed for the CLIENT on a Task Order
basis.
ARTICLE A: PURPOSE
This task order is to provide the City of Crestview with professional engineering services to
develop construction documents for the proposed Oakdale Parking Lot, located southwest of the
intersection of Oakdale Avenue and S. Ferdon Boulevard.
ARTICLE B: SCOPE OF WORK
Task 1- Project Management and Coordination
The Scope of Work provided below identifies the tasks to be completed to provide construction
documents for the proposed parking lot. We have assumed that this design will be completed in
conjunction with the Main Street project.
We have assumed that all project management and coordination is covered in the main Crestview Main
Street proposal.
Construction Documents
Task 2 — Pre -Design Activities
The Matrix team will complete a series of data collection activities upon receiving notice to proceed.
These data collection activities will include the following:
Task 2.1 Topographic Survey — We have assumed that topographic survey will be provided by the City
of Crestview.
Task 2.2 — Subsurface Utility Investigation — Matrix will follow the Subsurface Utility Investigation best
practices to determine the location of underground utilities within the limits of the project. We assume
potholes will be required. As part of our preliminary design. We have assumed that no additional
potholing or ground penetrating radar will be needed to identify underground utilities.
Task 2.3 - Geotechnical investigation — We have assumed that pavement design and geotechnical
investigation will not be required for the design of the parking lot. Our team proposes that we use design
from the Main Street project.
Task 3 - Construction Documents
Construction documents will be prepared for the submittal to the City of Crestview for infrastructure
improvements and submitted through the City's review process. Packages will be submitted to the city at
90% and 100% design. The following plans will be submitted:
Task 3.1— Parking Lot Plans — Matrix will prepare design for the parking lot (per City standards) based
on the design criteria. Matrix will include parking lot grading and drainage design for the parking lot.
Task 3.2 - Drainage Plans - Matrix will review the grading and drainage for the parking lot. We have
assumed that all drainage will be conveyed via curb and gutter to existing inlets, it is also assumed that
water quality will not be provided for the parking lot.
Task 3.3 - Erosion Control Plans - Matrix will provide erosion control plans for the project. The plans
will include erosion control measures that meet the requirements of the city as well as appropriate details.
Matrix will rely on the Contractor to apply for and obtain any state permits that may be required.
Task 3.4 - Specifications — Matrix will prepare technical specifications along with measurement and
payment specifications for all bid items required to construct the project as part of the final construction
document set. These will be prepared as revisions to FDOT specifications.
Task 3.5 - Cost estimate — Matrix will prepare an opinion of probable construction cost based on our final
design plans and specifications. Upon review of the opinion of probable construction cost, the project
team can determine if any scope items should be added or removed or if any bid alternatives should be
created.
Task 3.6 - Submit Review Design Packages — The project team will prepare a design package at the 90%
and 100% design levels consisting of the following items:
• Drainage letter
• Parking lot plans and grading design.
• Engineers Opinion of Probable Cost
• Construction phasing
These deliverables will be submitted to the City for review and approval, we assume any referrals will be
made by the City.
Task 3.7 - Design Review Meetings (3) — Matrix will schedule and conduct design review meetings where
all impacted stakeholders can attend and review their comments on the design package. Matrix staff will
take meeting notes to document the comments received and will prepare a design comment matrix with
responses for distribution. Stakeholders will have an opportunity to review the Matrix proposed
responses so that a consensus can be reached on how to address the design comments.
Task 4 - Engineering Reports
In addition to the construction documents, reports are required by the City to gain approval. Reports
include a drainage report, sanitary sewer report, and traffic impact study.
Task 4.1— Drainage Letter — Matrix will provide a drainage letter providing details regarding drainage
patterns and calculations for flows from the proposed parking lot.
Task 5 - Final Plans
Matrix will address any final comments that are provided from the review of the 100% design package
and submit for approval. Comments will be addressed and coordinated with the review agencies, and
final Construction Drawings (CD's) will be prepared and submitted to the City for approval. The CDs
will contain civil plans necessary for the City of Crestview to bid and construct the improvements for the
project.
Task 5.1— Final Plans: The Matrix team will prepare a final plan set for approval by the City, per the
City's standards.
Task 5.2 — Specifications: Our team will prepare final specifications for the project to be included in the
bid documents. Specifications will include measurement and payment for bidding.
Task 5.3 —Final Engineers estimate: Matrix will prepare a final opinion of probably construction cost
based on our final design plans and specifications.
Assumptions
• We have assumed that the City will provide all documents for standard City requirements
and contracts.
• Survey of parking lot area is excluded from the scope. We have assumed that base files will be
provided by the City of Crestview along with conceptual design and location of the parking lot
in AutoCAD format.
• Potholing or ground penetrating radar investigation of existing utilities is excluded from the
scope.
• Environmental Phase 1 and Phase 2 are excluded from the scope.
• Geotechnical investigation and pavement design are excluded from the scope.
• Water quality and detention will not be provided for drainage coming from the parking lot.
• All agency fees are excluded and shall be paid directly by the Owner.
• All work will be completed in AutoCAD and MS office format and as required by the
review agencies.
• Infrastructure design not stated in the above text is excluded.
• We have assumed that this work will be done concurrently with the Main Street project.
• Supporting documentation for financing or regulatory packages that are not specifically
referred to in the scope of services is excluded.
• Specific bidding support for this as a stand-alone project is excluded from the scope.
• Construction management, construction inspections and construction support services are
excluded.
• Improvements to utility infrastructure that is not identified in the scope above is
excluded.
• We assume that the plans will only be reviewed and approved by the City of Crestview.
ARTICLE C: COMPENSATION PROVISIONS
Total cost: $12,500 and is inclusive of all labor and expenses.
ARTICLE D: PERIOD OF SERVICE
We anticipate that the design of the parking lot will take approximately 60-90 days after notice to
proceed.
ARTICLE E: AUTHORIZED REPRESENTATIVES
The Authorized Representatives designated below are authorized to act with respect to Oakdale
Avenue Parkin:. Lot Task Order 2. Communications between the parties and between Matrix
Design Group subcontractors shall be through the Authorized Representatives:
