Subject: Out -of -Scope Letter Agreement — Pi Lloyd Street Lift Station
Dear Mr. Mayor.
Operations Management International, Inc. (OMI) would like to propose the following out -of -scope
services and materials:
This is for PI Lloyd Street Lift Station repairs. To perform this service the follow tasks will be
completed:
1. Install temporary piping and by-pass pumping.
2. Replace Floats (4) for level control.
3. Install floats on stainless steel chain so floats can be pulled for cleaning.
4. On each wet -well (3) the wet -wells will be cleaned so we can enter the well to remove
the section of suction piping to be replaced with stainless steel material.
5. The wet -wen to be worked on will have pipe plugs installed for isolation.
6. Remove old ductile iron suction line from wet -well.
7. Measure old ductile iron suction line so new stainless steel can be laid out and welded out
for installation.
8. Install new flange adapter to suction bell.
9. Remove all old materials and trash then clean station.
10. Test pump station equipment for proper operation.
NOTES:
When it is being provided: Start date to be determined
Project costs:
The estimated price of this work is $81,882.17. City agrees to pay OMI
on a cost basis plus 15% for services completed.
Payment terms: As defined in Article E.23 of the Agreement, payment will be due and
payable within fifteen (15) days following receipt of OMI's invoice.
All other terms and conditions of the Agreement executed October 22, 2014, between OMI and City of
Crestview, FL remains in full force and effect.

If these terms are agreeable to you, please sign this letter. A fully executed version of this Agreement
will be returned for your files.
OMI appreciates the opportunity to provide these additional services to the City of Crestview.
