TASK ORDER NO. 0008
This task order amendment is issued pursuant to the PROFESSIONAL ENGINEERING SERVICES
CONTINUING SERVICES CONTRACT dated May 2021 between SEASIDE ENGINEERING AND
SURVEYING, LLC, ("COMPANY") and THE CITY OF CRESTVIEW, FLORIDA ("CLIENT") for
services performed for the CLIENT on a Task Order basis.
ARTICLE A: PURPOSE
Introduction
The services to be rendered by the Company under this task order consist of providing professional
surveying services necessary to provide the following:
• Boundary, As -Built, Topographic survey, & Wetland Delineation of the City -owned property
identified as Parcel 28-3N-23-0000-0011-0000 by the Okaloosa County Property Appraiser, said
property is located at the intersection of Brookmeade Drive and Covell Road in Crestview. The
intent is for the survey to provide a clear picture of the existing conditions for design &
permitting of future improvements.
ARTICLE B: SCOPE OF WORK
Task 1
Boundary, As -Built, Topographic survey of the City -owned property identified as Parcel 28-3N-23-0000-
0011-0000 by the Okaloosa County Property Appraiser, said property is located at the intersection of
Brookmeade Drive and Covell Road in Crestview. The intent is for the survey to provide a clear picture
of the existing conditions for design & permitting of future improvements. All work shall be performed
under the supervision of a professional surveyor, licensed in the State of Florida.
Task 2
Wetland Delineation and Assessment Report performed by Wetland Sciences, Inc.
Data Deliverables shall include the following:
1. Two signed and sealed copies of the survey will be provided as well as a copy of the electronic
drawing file for use by the City (AutoCAD).
2. Copy of the Wetland Assessment Report in PDF format.

ARTICLE C: COMPENSATION PROVISIONS
Compensation to be paid to the Company for providing the requested Services for this specific project
shall be as follows:
A Lump Sum Fee of $14,500 for the services listed in Article B.
ARTICLE D: PERIOD OF SERVICE
Unless otherwise directed all work shall be completed and final submittals within 60 calendar day s
following task order award.
ARTICLE E: AUTHORIZED REPRESENTATIVES
The Authorized Representatives designated below are authorized to act with respect to Task Order No.
0008. Communications between the parties and between any subcontractors shall be through the
authorized representatives:
