All efforts have been made to assure that messages and files generated from this office are free and clear of this type of material. 1 TOWN OF GULF STREAM Overhead Utility Undergrounding Initiative SURVEY WORK ORDER No: 2 Work Scope for Survey Services Required for Phase 2 Date: February 23, 2012 Town will review and negotiate any issues or concerns. Upon successful completion of review and negotiation, Town will issue a notice to proceed. In the event negotiations are not successful, Town will move to next highest ranked provider. Surveyor is requested to provide a proposal including estimated days to complete, a not to exceed fee amount, and a breakdown by activity. The following shall apply. 1. Services provided via this work order shall be provided in accordance with the current Master Agreement. 2. The Town shall provide access to it's record relative to Data, Maps, and Drawings which might be beneficial in delivering the specified deliverables. 3. Surveyor shall invoice for services monthly and/or at the completion of the work. 4. Surveyor shall deliver two sets of drawings depicting required information on 24" x 36" sheets and one set in AutoCAD 2004 or later "dwg" format on media or via electronic method. 5. Utility company input is not considered to be a requirement for this work Order. If surveyor feels such contacts would be beneficial, he should propose such and explain the expected benefits. 6. To ensure that the Town and the surveyor are in agreement as to the deliverables, the Surveyor shall list all proposed tasks deemed necessary to complete the work and the estimated fees for each. 7. Fees for printed sheets shall be as provided for in the Master Agreement rate schedule. 8. All expenses including travel expenses shall be included in the proposal. 9. Drawings will include County Aerial Images or approved equal in a separate layer. The geographic boundaries for Phase 2 extend more or less from the north limits of that portion of the Town of Gulf Stream being east of the hntracoastal waterway southerly to Golfview Drive. Files shall be in an AutoCAD format version 2004 or higher in "dwg" format. Parcel Data and information for survey document obtained by Surveyor to be consistent with County Tax Assessor's data bases. Location and documentation of the following items within and along the right-of- ight-of- way wa are to be included as part of the survey document dtiverables: 1 Centerline and right-of-way lines of all roadways and streets.(where mentioned herein, roadways and streets shall include private and public) 2 Property Plat Information, Plat lines and property ownership lines, Lot and Block numbers, Parcel control nubers, addresses, road and street names. 3 Edges of pavement on all streets. Edges of driveway pavement/pavers.etc from street connection to 10 feet outside the street right-of-way. 4 Geographic tie down to the intersection of the centerlines of South Ocean and Golfview Drive. 5 Edges of Rambles / Sidewalks within right-of-ways or within 10 feet of the right- of-way, 5 Fixed structures and other existing above ground improvements excluding landscaping within road/street right-of-ways or within 20 feet of such right-of- ways. 7 All visible utilities within road right-of-ways or within 10 feet of road right-of- way, including but not limited to water meters, water valves, fire hydrant, Electric boxes, telephone boxes, poles, overhead wires, transformer pads, Tel/CATV pedestals, guying anchors, Resident owned lighting, culverts, ditches over 12" deep, drainage structures, private and public sewer system facilities, and TV Cable boxes. 8 Show all FPL, cable tv, and telephone overhead wires, poles, boxes, and equipment. FPL padmounted transformers not within road right-of-way have been depicted on the attached drawings. 9 Show all known easements known or made available by the Town. 10 Survey to terminus all overhead power lines and associated equipment that are located out of road right-of-way. Indicate all pole locations, down guys, transformers, and in -ground service/splice boxes. I l Provide examples of symbols to be utilized on drawings prior to beginning work for review by the Town. 12 Remote FPL Padmounted transformers have been located by the Town and general location information will be provided by the town for all such transformers not located adjacent to the right-of-ways.
