STANDARD AGREEMENT

This agreement made this 16 day of January in the year 2023 by and between:

Hereinafter called the Owner and Constantine Constructors, LLC. (COL), 1988 Lewis Turner Blvd Unit 3, Fort Walton
Beach, FL 32547 hereinafter called the contractor, witness that for the consideration named herein, agree as follows:
Article 1
The Owner agrees to furnish all labor, material and equipment required to perform all work as described in Article 11
hereof for the:
Removal of the existing bnck wall that was damaged by the Contractor's activities.
The contractor agrees to furnish all labor, material and equipment at no cost f$0.00) to the Owner to perform all work
as described in Article 11 hereof for the:
Installation of approximately 90 If of a new wooden fence that will replace the brick wall that was damaged by the
contractors' activities during the installation of the new 6" FM.
Article 11
The Owner and the contractor agree that the material and equipment to be furnished and work to be done by both the
Owner and Contractor are as follows:
This Owner will furnish all labor, material, and equipment to complete:
Remove and dispose the damaged section of the brick wall.
This Contractor and its subcontractor will furnish all labor, material, and equipment to complete:
Install approximately 90 If of wooden fence to replace the brick wall that was damaged by the Contractors
activities.

Amount of $400.00
For the clean up of my brick wall fallen from the installation of the sewer line installed by Constantine
Constructors, LLC for the City of Crestview. The employees of Constatine Constructors moved the dirt
from the wall on my property at 205 Golf Course Dr. and disturbed the ground causing the collapse of
my wall. I am paying to have the brick removed, this is a reimbursement to me.
