John f Acct # _14003049 PREMIUM AGREEMENT COVERS THE FOLLOWING: • Lawn Destroying Insect Control: includes such lawn destroying insects as mole -crickets, chinch bugs, spittle bugs, and sod web worms. • Disease Control: includes Brown Patch, Grey Leaf Spot and Dollar Spot fungus. Disease such as Pythium Blight and Take -All Root Rot are not included and would require additional cost. • Fertilization: nutritional packages throughout the year. • Shrub Program: consists of insect control, disease control and fertilization as needed throughout the year. • Weed Control: Pre -emergent herbicides and spot treatments of post -emergent herbicides to turf only. Post - emergent applications will be applied only when conditions are favorable without doing damage to your turf. Unfavorable conditions include: drought stress, high temperatures, disease stress, insect stress and mechanical stress, such as mowing too short or de -thatching. It is understood certain weeds cart only be suppressed. NOTES: Insects such as ants, fleas and ticks are not considered Lawn Destroying and will not be included in your service. Cycad scale is not included on shrubs and ornamentals. However, these insects will be addressed upon request at a reduced additional rate. The control of nematodes and ground pearls are not included in this service as well as mammals such as armadillos and moles. Bryan Pest Control also offers 'as needed' fawn services, but not limited to: • Aeration: Removes small plugs of thatch and soil, thus leaving small holes in the soil that allow penetration by air, water, fertilizers, and pesticides. • Soil Testing: Soil samples will be taken from your yard and lab -tested for deficiency. • Lime: Lim e applied to correct PH level. • Once a Year Fire Ant Control These services will be offered as needed and will be quoted at customer's request. Disease or damage to lawns or landscape plants caused by excessive irrigation, lack of water, or acidity. 2. If owner waters at the incorrect time, i.e., too much, too little, improper coverage. --3.---Improper mowing height too law or scalping and mowing frequency, -dull mower -blades mowing whengrass--- is wet. 4. Cold, flooding, wind or storm damage. 5. Thatch buildup. 6. High traffic areas of lawn. 7. Unfavorable conditions for weed control which includes drought stress, disease stress, insect stress or other stress including excessive watering. 8. Products applied by customer that can damage turf, i.e., weed and feed, herbicides, fertilizers. Many factors are beyond the control of Bryan Pest Control which have a direct effect on the quality and life of your landscape. Bryan Pest Control cannot be responsible for the loss of turf or plants pertaining to these factors. We service once a month and need the customer as a teammate during the time between services. Please notify us of any issues in your landscape. The customer agrees to provide proper maintenance to the landscape, and to call Bryan Pest Control when possible issues arise between our visits. Bryan Pest Control will provide additional inspections upon request and treat at no additional cost if it is deemed covered under the service agreement. Our intent is to keep your lawn as healthy as possible. Please let us know when you see a problem. If our technicians are not courteous or do not take time to service your lawn to your satisfaction, please let us know. We appreciate your business and we want to fulfill our obligation to you. It is important that, together with you, we function as a team. We will come once a month and advise you as much as we can. If issues arise between our scheduled visits, we will make additional stops at no charge upon request by the owner. This agreement is renewable from year to year unless thirty (30) days' notice is given by either party. If paid in full prior to the first treatment, we will render a five (5) percent discount. The yearly amount is subject to change annually with written notification or with any changes to the size of the treatment area.
