RESOLUTION APPROVING FAMILY LEAVE
WHEREAS, the Family and Medical Leave Act, 29 U.S.C. Section 2601,
et. seq. (the "Act"), allows employees to take unpaid leave from
their employment to care for certain family members; and
~, an employee of the Borough of Carteret (the "Borough")
has requested leave to care for a family member covered by the
Act; and
~, the Borough wishes to grant said request.
NOW, ~RE, BE IT RESOLVI~), by the Mayor and Council of the
Borough of Carteret, that Michael Majur be hereby granted Family
Leave for a period not to exceed twelve (12) weeks.
BE IT FURTH}~ RESOLV~I), that said leave shall be effective Tuesday,
September 7, 1999.
BE IT F~ RESOLV}~), that this leave is conditioned upon Mr.
Majur's compliance with the Act, ad the New Jersey Family Leave
Act, and production of a note from the affected family member's
physician indicating that the family member suffers from a serious
health condition (as defined in the Act) and that Mr. Majur's
assistance is required in caring for the affected family member.
