FOR
ADDITIONS, DECKS, PORCHES, WHEN BUILDING FOOTPRINT IS EXPANDING
1. (3) Sets of plans* (with elevations and framing details). Applications for additions must include
proposed and existing floor plans of the entire building, Stamped by Registered Design Professional
when required, (Architect or Engineer), *One (1) large set (18’ x 24” or larger) and two (2) 11”x17.”
FLOOR PLANS MUST INCLUDE LOCATION OF ALL REQUIRED FIRE PROTECTION SYSTEMS, HEATING SYSTEM
COMPONENTS AND STORAGE AREAS BEING ADDED. (NOTE: Entire Fire Protection System must be upgraded to
current code if a bedroom is being added). A COPY OF THE PLAN WILL BE FORWARDED TO THE BREWSTER FIRE
DEPARTMENT FOR REVIEW.
1. (1) Site Plan (drawn and stamped by surveyor or engineer). Include location of existing and proposed
setbacks, all utilities, lot coverage calculations, setbacks, all overlay districts
2. Energy Calculation (IECC 2012 ResCheck) Include # of Square Feet and Window Ratings
3. Septic Permit or Waiver Issued by Brewster Health Dept. (if applicable) – (Filed Separately)
4.MA Checklist for Compliance (780 CMR 5301.2.1.1) to meet Wind Code Requirements
5.Copy of Current Worker’s Compensation Insurance Certificate (if applicable)
6.Copy of Current MA C.S.L. & H.I.C. Registration (if applicable), signed Homeowner Authorization Form
or completed Homeowner’s License Exemption Certificate
7.Completed and signed Debris Disposal Affidavit (State Requirement)
8.Copy of MA Construction Supervisor’s license and Home Improvement Contractor’s Registration, with
numbers and expiration dates, or Homeowners Exemption Certificate and Affidavit
9.Copy of Worker’s Compensation Certificate of Insurance
10.RECORDED COPY of Conservation Commission Order of Conditions or copy of Negative
Determination documents (if applicable). (NOTE: Any construction within 100 ft. of a Conservancy
District is under the jurisdiction of the Conservation Commission.)
11. RECORDED COPY of ZBA Special Permit or Variance, or Planning Board Statement of Conditions (if
applicable)
NOTE: A FOOTING FORM INSPECTION IS REQUIRED ON ALL SONOTUBE INSTALLATIONS. You must
call the Building Department to schedule your inspection before concrete is poured. Failure to do so will result in a failed
inspection, and excavation of all poured footings.
