FOR
NEW CONSTRUCTION
1. (3) Sets of plans, including framing details for all construction (including decks, porches, etc.)
Stamped by MA Registered Design Professional when required, (Architect or Engineer), Please
include One (1) large set (18” x 24” or larger and two (2) 11x17 sets.
FLOOR PLANS MUST INCLUDE LOCATION OF ALL REQUIRED FIRE PROTECTION SYSTEM
COMPONENTS, HEATING SYSTEM COMPONENTS AND STORAGE AREAS. A COPY OF THE PLAN
WILL BE FORWARDED TO THE BREWSTER FIRE DEPARTMENT FOR REVIEW.
2. (1) Site Plan (drawn and stamped by surveyor or engineer). Include location of existing and
proposed driveways, all utilities, lot coverage calculations, setbacks, all overlay districts
3. Completed Curb Cut Permit Application (if applicable)
4. Septic Permit or Waiver Issued by Brewster Health Dept. (if applicable) – (Filed Separately)
5. Completed and signed Debris Disposal Affidavit (State Requirement)
6. Completed and signed Worker’s Compensation Insurance Affidavit (State Requirement)
7. Separate stamped structural engineering designs for all engineered lumber and materials
8. Energy Calculations (2018 IECC ResCheck, signed) or HERS Rater Report (Please Include # of
Square Feet, Glazing Areas and Window Ratings)
9. MA Checklist for Compliance (780 CMR 5301.2.1.1) to meet Wind Code Requirements
10. Separate Door and Window Schedule (unless listed separately on plans).
11. Copy of Current MA C.S.L. & H.I.C. Registration (if applicable), signed Homeowner
Authorization Form or completed Homeowner’s License Exemption Certificate
12. Copy of Current Worker’s Compensation Insurance Certificate (if applicable)
13. RECORDED COPY of Conservation Commission Order of Conditions or copy of Negative
Determination documents (if applicable). (NOTE: Any construction within 100 ft. of a Conservancy
District is under the jurisdiction of the Conservation Commission.)
14. RECORDED Copy of Special Permit or Variance from Zoning Board of Appeals or Planning
Board Statement of Conditions (if applicable)
