Stream and her name is
Kelly Avery.
Can you please supply for date of hire, separation date, performance reviews,
commendations and/or disciplines, and reason for separation?

The Town of Gulf Stream has received your public records request dated February 11, 2020. You

Kelly Avery was hired by the Town of Gulf Stream on October 31, 2011 and her separation date
was June 17, 2016. The Town does not do performance reviews and there are no
commendations and/or disciplines in the files. A copy of the letter of resignation is attached.
