Good afternoon,
Attached is an information request for a document like the other document attached. The internal affairs
investigative reports’ verifications pursuant to Fla. Stat. 112.533(1)(a) should be on a single piece of paper that
is the very last page of every Internal Affairs investigative report that’s been done with regard to a complaint
against a sworn law enforcement officer and/or certified corrections officer. Someone in professional standards
or an internal affairs unit would be familiar with that page and be able to assist in locating one. A template of
the page is fine if there is any concern about officers names or personal being on it.

the subject statutorily requisite verifications pursuant to §§ 112.533(1)(a)1 and 112.533(1)(a)2
appear.
If you reasonably anticipate that the actual cost for producing the requested
records will exceed $2.00 please advise the undersigned in advance of incurring
any costs.
While we believe that the record(s) requested above are not exempt from inspection,
production and/or copying, if you assert that an exemption applies to all or part of a particular
public record requested, please state in writing the statutory citation to the exemption(s) you
claim applies and, with particularity, the reason(s) for the conclusion that any requested record
or portion thereof is exempt or confidential. See Fla. Stats. 119.07(1)(e) and (f). If you assert
that an exemption so identified applies only to a portion of a public record, pursuant to Fla. Stat.
119.07(1)(d) you are required to redact from the record only that portion with respect to which
