RESOLUTION NO. 1555
A RESOLUTION APPOINTING THE SOUTHWEST AGENCY
FOR RISK MANAGEMENT (SWARM) AND SOUTHWEST
AGENCY FOR HEALTH MANAGEMENT (SWAHM)
DESIGNATE AND ALTERNATE REPRESENTATIVES
BE IT RESOLVED BY THE VILLAGE OF PLAINFIELD, WILL AND
KENDALL COUNTIES, ILLINOIS, that Joan Zelinski, Director of Human Resources,
be hereby appointed as the Village of Plainfield SWARM Representative and that, Traci
Pleckham, Director of Management Services, be hereby appointed as the Village of
Plainfield SWARM Alternate Representative.
BE IT FURTHER RESOLVED that Joan Zelinski, Director of Human
Resources be hereby appointed as the Village of Plainfield SWAHM Representative and
that Traci Pleckham, Director of Management Services, be hereby appointed as the
Village of Plainfield SWAHM Alternate Representatives.
