A RESOLUTION APPOINTING THE SOUTHWEST AGENCY
FOR RISK MANAGEMENT (SWARM) AND SOUTHWEST
AGENCY FOR HEALTH MANAGEMENT (SWAHM)
DESIGNATE AND ALTERNATE REPRESENTATIVES
SE IT RESOLVED BY THE VILLAGE OF PLAINFIELD, WILL AND
KENDALL COUNTIES, ILLINOIS, that Traci Pleckham, Director of Management
Services be hereby appointed as the Village of Plainfield SWARM Representative and
that Joan Zelinski, Director of Human Resources, be hereby appointed as the Village of
Plainfield SWARM Alternate Representative.
BE IT FURTHER RESOLVED that Christopher A. Minick, Village
Administrator be hereby appointed. as the Village of Plainfield SWAHM Representative
and that Traci Pleckham, Director of Management Services and Joan Zelinski, Director of
Hunr~an Resources, be hereby appointed as the Village of Plainfield SWAHM Alternate
Representatives.
