RESOLU'T'ION NO 1217.,__
A RESOLUTION ADOPTING A SUPPLEMENTAL INSURANCE PLAN FOR THE
EMPLOYEES OF THE VILLAGE OF PLAINFIELD
WHEREAS, The Village of Plainfield is committed to offering its employees a wide
array of benefits, and
WHEREAS, A supplemental line of insurance coverage is available through AFLAC,
and
WHEREAS, The Board of Trustees of the Village of Plainfield has ascertained that it
would be beneficial to offer the line of supplemental insurance available through AFLAC
to its employees, and
WHEREAS, Each employee of the Village of Plainfield shall have the option of
participating in the supplemental insurance line at the employee's cost, and
WHEREAS, the premiums for the line of supplemental insurance can be deducted on a
pre-tax basis from each participating employee's pay,
NOW THEREFORE BE IT RESOLVED BY THE BOARD OF TRUSTEES OF THE
VILLAGE OF PLAINFIELD, that the line of supplemental insurance coverage offered
by AFLAC be offered to all full-time employees of the Village of Plainfield and that all
necessary meetings be held so that the supplemental insurance coverage selected by
employees is effective as of February 1, 1999.
