A RESOLUTION AUTHORIZING A CHANGE ORDER IN THE CONTRACT
BETWEEN THE VILLAGE OF PLAINFIELD AND "D" CONSTRUCTION.
WHEREAS, the Village of Plainfield and "D"Construction previously entered into
a contract for the construction of improvements on Van Dyke Road; and
WHEREAS, it has been determined that it is in the best interest of the Village
to add certain work to the original contract; and
WHEREAS, the circumstances necessitating said change were not reasonably
foreseeable at the time the contract was executed and said change order is in the best
interest of the Village of Plainfield, as authorized by law.
NOW THEREFORE BE IT RESOLVED BY THE PRESIDENT AND BOARD OF
TRUSTEES OF THE VILLAGE OF PLAINFIELD, WILL COUNTY, ILLINOIS AS FOLLOWS:
That it is hereby determined that it is in the best interest of the Village
of Plainfield to authorize Change Order Na. 1, dated July 27, 1998, and
that the circumstances necessitating said change were not reasonably
foreseeable at the time the contract was executed and that said change
order is in the best interest of the Village of Plainfield, as authorized by
law.
II. The President is hereby authorized to execute said proposed change
order, a copy of which is attached hereto and made a part hereof as
Exhibit "A", and that a copy of said Resolution and Exhibit shall be
preserved in a file regarding said contract which shall be open to the
public for inspection pursuant to 720 Illinois Compiled Statutes 5/33E-9.

Our Project No. 79376
Members of the Board:
Please find enclosed Pay Request 3 and Change Order 1 for the captioned project. Pay Request 3
is in the amount of $213,581.44. The quantities are agreed to final quantities as of this date.
Additional fall seeding may increase items 30, and 32-36. 10~ retention is currently being
withheld. Change Order 1 is an increase in the amount of $19,484.56. The majority of this change
order is due to items 48, Extra 1 and 5, which were due to poor soils beneath the road.
We recommend your favorable consideration of both the pay request and change order.
The check should not be released to the contractor until IDOT Form BC-141 has been submitted
by the contractor.
