BE IT RESUVt.ED, by the PIyATNE.T-ELD BOARD „Q~$[,U~~;FFE _., , of the

VILLAGE of PLAINFIELD ,Illinois, that there is hereby
(Cily, Town or Village) (Name)
appropriated the sum of s 40, 700.00 „ of Motor Fuel Tax funds for the purpose of maintaining
streets and highways under the applicable provisions of the Illinois Highway Code, from January 1, 19.3,2
to December 31, 19 97
BE IT FURTHER RESOLVED, that only those streets, highways, and operations as listed and described on the ap-
proved Municipal Estimate of Maintenance Costs, including supplemental or revised estimates approved in connec-
tion with this resolution, are eligible for maintenance with Motor Fuel Tax funds during the period as specified above.
BE IT FURTHER RESOLVED, that the Clerk shall, as soon as practicable after the close of the period as given
above, submit to the fepartment of Transportation, on forms furnished by said Department, a certified statement
showing expenditures from and balances remaining in the account(s) for this period; and
BE -7 FURTHER RESOLVED, that the Clerk shall immediately transmit two certified copies of this resolution
to the district office of the Department of Transportation, at SCHAUMBT~RG„ __ ,,_.,_,., , -Ilinois.

The resolution adopted by the President and Board of Trustees on November 4, 1996
appropriating $40,700.00 of motor fuel tax funds for the maintenance of various
municipal streets from January 1, 1997, to December 31, 1997, was approved by this
Department on November 19, 1996.
The detailed estimate of maintenance cost in the amount of $40,700.00 for municipal
streets was also approved by this Department on November 19, 1996. Your file copy
is being returned herewith and to Dave Countryman, Director of Fublic Works.
Expenditure for the full amount of the approved estimate $40,700.00 from your
unobligated motor fuel tax account is hereby authorized and will be recorded on your
next monthly Allotment Notification Letter.
