A RESOLUTION AUTHORIZING THE EXECUTION OF A CONTRACT WITH C.
THOMPSON & SONS, INC. FOR THE JEC-LICHTENWALTER WATER AND
SEWER EXTENSION.
NOW, THEREFORE, BE IT RESOLVED BY THE PRESIDENT AND BOARD OF
TRUSTEES OF THE VILLAGE OF PLAINFIELD, AS FOLLOWS:
I: That the Village President is hereby authorized to execute and
the Village Clerk is hereby authorized to attest to the
execution of a contract with C. Thompson & Sons, Inc, for the
JEC-Lichtenwalter water and sewer extension.

10.2 Certificate by Applicant(s)
Commencing January 1,1990, Section 16.1 of the Environmental Protection Act (Ill. Rev. Stat.1987,
Ch. 111 1/2, par. 1016.1, as amended by P.A. 86-670) requires the Agency to collect a fee far certain applica-
tions for the installation or extension of water mains. There are no permit fees for other improvement to
public water supply systems and only certain water main projects will be affected.
In accordance with the Act effective January 1,1990, UWe hereby agree to pay the approporiate fee
for this Permit to install or extend Watermain. Except for the conditions listed below in 10.3 the following
fee schedule shall apply:
Fee Length of Watermain
xX $ 0 `l00 feet or less
$120 Greater than 200 feet, but not more than 1000 feet
$360 Greater than 1000 feet, but not more than 5000 feet
$600 greater than 5000 feet
Please check the appropriate fee; make cheek or money order payable to: "Treasurer, State of Illinois"
and submit along with this application. Any fee, remitted to the Agency shall not be refunded at any time or
for any reason, either in whole or in part.

b) Any unit of Loca] Government where all of the following conditions are met;
1)The cost of the installation or extension is paid wholly from monies of the unit of local
government, state grants or loans, federal grants ar loans, or any combination thereof.
2}The unit of local government is not given monies, reimbursed or paid, either in whole ar
in part, by another person (except for State grants or loans or federal grants ar loans;
and
