THIS ADDENDUM, (Number 1 to Contract No. 111-2023) is made and entered into 1 his
day of A''' I il , 2024, by and between Richmond, Indiana, a municipal corpora ion
acting through its Boa • of Public Works and Safety with its office at 50 North 5111 Street, Richnuind,
Indiana, 47374 (hereinafter referred to as "City"), and Cummins Electric Inc., 3712 National Road 'est,
P.O. Box 638, Richmond, Indiana, 47375 (hereinafter referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 111-2023 on June 14, 2023, w ich
Agreement retains Contractor to provide services in connection with the third-p rty
installation services of the 120v receptacle for the baby box safety device at Fire Sta ion
No. 1 for the Richmond Fire Department of the City("Project"); and
WHEREAS, The Parties have agreed that additional materials and labor are needed in orde to
properly relocate the existing outlet and to install an additional conduit and outlet for the
alarm associated with the baby box safety device, which services should be performed by
Contractor.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained, inclu ing

the above stated recitals, City and Contractor hereby agree as follows:
1. The City arid Contractor executed and entered into Contract No. 111-2023, on June 14,
2023, which Agreement is incorporated herei:n by reference. Pursuant to Contract .o.
111-2023, Section XI (Miscellaneous), the Parties may alter or amend the Agreemer in
whole or in part at any time by filing with the Agreement a written instrument setting
tbrth such changes signed by both Parties. The Parties agree the that Contractor shall
provide additional materials and labor needed in order to properly relocate an existing
outlet and to install an additional conduit and outlet for the alarm associated with the
baby box safety device all of which shall be in accordance with the scope of aid
Agreement. Said additional services shall be completed and delivered in a mariner

consistent with Contract No. 111-2023, which is hereby incorporated by reference ;Ind
made a part hereof. Compensation paid to Contractor as described in accordance ‘Oth
Section III (Compensation) of said Agreement shall be increased by an amount not to
exceed Seven Hundred Twenty-five Dollars and Zero Cents ($725.00) as listed in
Contractor's supplemental quote attached hereto as Exhibit A. The new total amount to
be paid to Contractor under the Agreement shall be an amount not to exceed Two
Thousand Four Hundred Dollars and Zero Cents ($2,400.00) for satisfactory
completion of the Agreement with the additional services and increase in scope as
described herein.
2. City and Contractor expressly agree that all other terms, conditions, and coven nts
contained in Contract No. 111-2023, shall be applicable to this Addendum, unless ich
term, condition or covenant conflicts with this Addendum Number 1, in which case his
Addendum Number I shall be controlling. ,

3. Both Parties hereby agree that with the exception of the additional materials and labor
needed as described herein, and with the exception of the corresponding compensation
increase, Contract No. 111-2023 shall remain intact and unchanged and in full force and
effect.
4. Both Parties agree that any person executing this Contract in a representative capacity
hereby warrants that he or she has been duly authorized by his or her principal to execute
this Acklendum.
IN WITNESS WHEREOF, this Addendum No. 1 to said Agreement executed and approved by

Re: Station #1 Baby Box
We propose to provide labor, tools, material, and supervision as required to move the outlet and add an extra
conduit for the baby box at Station #1 for Seven Hundred Twenty-Five Dollars ($725.00)
Scope of Work:
1. Move the outlet for the baby box up under the box then we can hide the cord.
2. Set a new 4 square box for the new outlet and put a blank cover on the old 4 square box.,
3. Run W EMT from the Electrical Room to the North side of the baby box for the alarm corrlpany.
4. Drill through the block wall to get the conduit from the Electrical Room to the Hallway.
5. Provide and install a plastic bushing on the end of the conduit in the Electrical Room and by the Baby
Box to protect the Low Voltage Cable getting pulled through the conduit.
Notes:
1. Proposal is based upon your request.
2. Work to be performed during normal business hours, Monday through Friday, 7:00AM to ':30PM

All work completed in a workmanlike mariner per standard practices. Changes from original
specifications that incur additional cost will become an extra charge to the original proposal. Our
workers are fully covered by Workmen's Compensation Insurance, Owner's to carry fire, tornado
and other necessary insurance. Proposal may be withdrawn if not accepted within 30 days.
Payment terms net 30 days after receipt of invoice.
