product approval, temporary and final certificates of occupancies related to all
construction, improvements, repairs, and modifications of the Subject Property.
If your office cannot comply with this request due to privilege, confidentiality or any
other reason, please provide our office with the particular reasons that prevent you from
complying with this request in writing, as required by Section 119.07(2)(a). If you claim
that any exceptions apply to a portion of the records only, please omit production of that
portion and provide copies of the remaining records.
We agree to pay the reasonable cost of duplication as defined in Section
119.07(1)(a). However, please contact our office prior to incurring in any expense to

Application is hereby made to obtain a permit to do the work and installation as indicated. I certify that no work or installation has
commenced prior to the issuance of a permit and that all work will be performed to meet the standards of all laws regulating
construction in this jurisdiction A City Building Permit does not assure compliance with Homeowners Association's rules,
regulations and/or deed restrictions. Applicant is advised to obtain approval from the Homeowners Association before
improving any property. Applicant further acknowledges the following:
Separate permits must be secured for ELECTRICAL, PLUMBING, MECHANICAL, WELLS, POOLS, FIRE SPRINKLER,
SIGNS, LANDSCAPE, IRRIGATION, ROOFING, SHUTTERS, ETC.
This permit becomes null and void if work or construction authorized is not commenced within six (6) months or it
construction or work is suspended or abandoned for a period of six (6) months at any time after work is commenced.
Permit will be considered suspended or abandoned if it does not pass an inspection within 180 days and will be subject to
a reactivation fee in the amount in force at the time of reactivation.
• Failure to comply with all applicable construction regulations may result in the withholding of future permits.
Submission of any false information or misrepresentation is a violation of law and shall result in revocation of your permit.
NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that
may be found in the public records of this county, and there may be additional permits required from other governmental
entities such as water management districts, state agencies, or federal agencies.
WARNING TO OWNER: YOUR FAILURE TO RECORD A NOTICE OF COMMENCEMENT MAY RESULT IN YOUR

Soilprobe Engineering & Testing, Inc. has completed field observation of steel helical piles
installation for the above referenced project. Piling operation was conducted by American
Foundation on March 12, 2019 and installation witnessed by a representative of this office. The
purpose of this site inspections was to observe and document piles installation operation only.
A total number of sixteen (16) 3" O.D. schedule 40 steel helix piers were placed at locations staked
in the field by the owner's representative and to depths as indicated in the attached field inspection
log. Pile installation data for each pile are presented in the attached field installation log.
Following our observation of pile installation operation at the scheduled time. and our knowledge of
project documents we certify that the referenced piles were adequately installed for achieving an
allowable bearing capacity of 10 tons compression.

Section 434.2.17 of the 2001 Florida Building Code and Chapter 515 of Florida Statutes
requires the provision of specific pool barriers around the entire perimeter of the pool.
In addition to this executed affidavit, all provisions for the required pool barrier shall be
shown on the plans, and manufacturer's product compliance information must be
supplied.
I (We) acknowledge that a new swimming pool, spa or hot tub will be constructed or
installed at the above address, and hereby affirm that one or more of the following
methods will be used to meet the requirements of the Florida Building Code, Section
424.2.17 and Florida Statutes, Chapter 515.
Please initial all applicable methods:
The pool will be equipped with an approved safety pool cover that complies with
ASTM 1346-91, Standard Performance Specifications for Safety Covers for
Swimming Pools, Spas and Not Tubs.
A continuous barrier ("Kiddie Fence") will be used to isolate the pool from the
home. Plans submitted show the location of the end, which is not removable
without the use of tools.
A continuous barrier such as a fence or screen enclosure shall be used to prevent
external access to the pool.
All doors and windows leading to the pool from the interior of the home that
meet 434.2.17, shall have an exit alarm meeting the requirements of UL 2017.

construction in this jurisdiction. A City Building Permit does not assure compliance with Homeowners Association's rules,

• Separate permits must be secured for ELECTRICAL, PLUMBING, MECHANICAL, WELLS, POOLS, FIRE SPRINKLER,
SIGNS, LANDSCAPE, IRRIGATION, ROOFING, SHUTTERS, ETC
• This permit becomes null and void if work or construction authorized is not commenced within six (6) months or if
construction or work is suspended or abandoned for a period of six (6) months at any time after work is commenced
Permit will be considered suspended or abandoned if it does not pass an inspection within 180 days and will be subject to
a reactivation fee in the amount in force at the time of reactivation.
• Failure to comply with all applicable construction regulations may result in the withholding of future permits.
• Submission of any false information or misrepresentation is a violation of law and shall result in revocation of your permit.
• NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that
may be found in the public records of this county, and there may be additional permits required from other governmental
entities such as water management districts, state agencies, or federal agencies.
WARNING TO OWNER: YOUR FAILURE TO RECORD A NOTICE OF COMMENCEMENT MAY RESULT IN YOUR
PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. A NOTICE OF COMMENCEMENT MUST BE
