Dear Rita:
This letter is a follow up to the information you provided in response to my previous public
records request dated June 18, 2013 (known as item "J "). We need to be able to match the
Town's land line and cell phone numbers with the employees associated with those numbers.
Therefore, on behalf of my client, Christopher O'Hare, and pursuant to the Public Records Act,
Chapter 119 of the Florida Statutes, I am herewith making a public records request for copies of
the following items:
1. A list, log, assignment sheet a• am other record shat documents n!I the celhrinr and
land line telephone numbers used hl• Torre employees in the coarse of their
emphrment and the employees to whonn these numbers were assigner! between 10 -27 -.1
11 and 10- 29 -11; and
2. A list, log, assignment sheet or ant• other record that documents all the cellular and
lard line telephone mmnhers reset! bl• Town employees in the coarse of their
ennplol•nnernt and the employees to rvlronr these numbers were assigned on 12- 28 -11;r
and
3. A list, log, assignment sheet or am other record that documents all the cellular and
land line telephone nunnhers used hp Town enrplal'ees in the coarse of their
ennplorrnent and the employees to whom these nunnhers were assigned on 3 -5 -12, and
4. A list, log, assignment street or am other retort! that documents all the cellular and

an15- 16 -13; and
8. In the alernatim, a single list, log, assignment street or am other record that
docuannents all the cellular and land line telephone nunrhers used by Town employees
and the ennplgrees to whom these nunrhers are assigned if said list, log, assignment
street or other record of said nunrhers and assigned ennphrees is the sane for all the
above requested tune periods.
If you refuse to provide this information, Chapter 119 requires that you advise me in writing and
indicate the applicable exemption to the Public Records Act. Also, please state with particularity
the reasons for your decision, as required by Section I I9.07(2)(a). If the exemption you are
claiming only applies to a portion of the records, please delete or redact that portion and provide
photocopies of the remainder of the records, according to Section I I9.07(2)(a).
If you can provide an estimate of the cost of duplication, I would appreciate it. If acceptable, 1
will pay the actual cost of duplication as defined in Section I I9.07(1)(a). However, if you
anticipate that in order to satisfy this request, "extensive use" of information technology
resources or extensive clerical or supervisory assistance as defined in Section 119.07(1)(b) will
be required, please provide a written estimate and justification.
I would ask that you provide all the above requested records no later than this coming
MONDAY. JULY 18.2013.
