O'Hare, I wish to make a public records request of your agency for the following records: A list, log, assignment skeet or any other record that documents all the cellular and land line telephone numbers used by Town employees in the course of their employment and the employees to whom these numbers were assigned between 10 -27 -11 and 10- 29 -11. All responses to this public records request should be made in writing, first to the following email address: lou@louroeder.com . or in the alternative to fax number: 866 -610 -6090 If the public records being sought are maintained by your agency in an electronic format, please produce the records in the original electronic format in which they were created or received. See §119.01(2)(0, Florida Statutes. If you contend that any of the records I am seeking, or any portion thereof, are exempt from inspection or disclosure, please cite the specific exemption as required by §199.07(1)(e) of the Florida Statutes and state in writing, and with particularity, the basis for your conclusions as required by §119.07(1)(0 of the Florida Statutes. Please take note of §119.07(c) Florida Statutes and your affirmative obligation to (1) promptly acknowledge receipt of this public records request and (2) make a good faith effort which "includes making reasonable efforts to determine from other officers or employees within the agency whether such a record exists and, if so, the location at which the record can be accessed." I am, therefore, requesting that you notify every individual in possession of records that may be responsive to this public records request to preserve all such records on an immediate basis. If the requested records are not all readily available, I will accept them piecemeal. I request that whichever records are readily available be provided to me immediately; and those other records be provided to me as soon as possible. If you anticipate the production of these public records to exceed $1.00, please notify me in advance of their production with a written estimate of the total cost. We need to be able to match the Town's land line and cell phone numbers with the employees associated with those numbers. Therefore, on behalf of my client, Christopher O'Hare, and pursuant to the Public Records Act, Chapter 119 of the Florida Statutes, I am herewith making a public records request for copies of the following items: A list, log, assignment sheet or a ?y otter record that docuntents all the cellular and hard line telephone numbers used by Town employees in the coarse of their entployntent and the employees to whom these numbers were assigner! between 10 -27- 11 and 10- 29 -11; and 2. A list, log, assignment sheet or any other record that documents all the cellular and land line telephone numbers used by Town employees in the course of their emtployment and the employees to whom these numbers were assigned on 3 -5 -12; and 4. A list, log, assignment sheet or mq• other record that documents all the cellular and land line telephone numbers used by Town employees in the course of their employment and the employees to whom these numbers were assigned between 10 -1 -12 and 11 -5 -12; and 7. A list, log, assignment sheet at- any other record that documents all the cellular and load lire telephone numbers used by Town employees in the course of their employment and the employees to whom these numbers were assigned hetweeu 2 -26 -13 and 5- 16 -13; and 8. 6c the alternative, a single list, log, assignment sheet a• any other record that documents all the cellular and land line telephone numbers used by Town employees and the employees to whom these numbers are assigned if said list, log, assignment sheet or other record of said numbers and assigned employees is the same for all the above requested time periods. If you refuse to provide this information, Chapter 119 requires that you advise me in writing and indicate the applicable exemption to the Public Records Act. Also, please state with particularity the reasons for your decision, as required by Section I I9.07(2)(a). If the exemption you are claiming only applies to a portion of the records, please delete or redact that portion and provide photocopies of the remainder of the records, according to Section 119.07(2)(a). If you can provide an estimate of the cost of duplication, I would appreciate it. If acceptable, l will pay the actual cost of duplication as defined in Section I I9.07(1)(a). However, if you anticipate that in order to satisfy this request, "extensive use" of information technology resources or extensive clerical or supervisory assistance as defined in Section I I9.07(1)(b) will be required, please provide a written estimate and justification. I would ask that you provide all the above requested records no later than this coming MONDAY, JULY 18, 2013.
