Thank you in advance for your assistance. In accordance with Florida Statute Chapter 119, please consider this a formal
request for public records. Please send digital copies of the following items regarding your entity’s property and
casualty insurance program to the email address: 048.information@publicrisk.com. Please include the following:
-Your current (2022/2023) FMIT insurance policy; including all declaration pages, forms, and premiums; for Property,
General Liability, Auto Liability and Physical Damage, Workers Compensation, Public Officials Liability, Employment
Practices, Liability, Law Enforcement Liability, Cyber Liability and Crime.
-Five-year detailed claims history for the lines of coverage referenced above.
Please advise of any fee associated with this request, and we will be happy to provide in advance of any burden upon
you.

Thank you in advance for your assistance. In accordance with Florida Statute Chapter 119,
please consider this a formal request for public records. Please include the following:
-Your current (2022/2023) FMIT insurance policy; including all declaration pages, forms, and

The Town of Gulf Stream has received your public records request dated April 14, 2023. The
original public records request can be found at the following link:

The Town now estimates that to fully respond to your request will require approximately one
hour of administrative support at $67.41 per hour, the labor cost of the personnel providing the
service, per Fla. Stat. § 119.07(4)(d). If the costs of producing these documents will exceed
your deposit, the Town will provide you with an initial production of responsive records and an
estimate for the production of any additional responsive records. If the costs of production are
less than the deposit, the Town will provide you with the responsive records and a refund.
(One hour @ $67.41) = Deposit Due: $67.41 in cash or check.
Upon receipt of your deposit, the Town will use its very best efforts to further respond to your
public records request in a reasonable amount of time. If we do not hear back from you within
30 days of this letter, we will consider this request closed.
