THIS IS A PUBLIC RECORDS REQUEST
ATTENTION: Custodian of Public Records for the Town of Gulf Stream, FL
Pursuant to Article 1, Section 24 of the Florida Constitution and Chapter 119.07
of the Florida Statutes, and on behalf of my client Christopher F. O'Hare,
I wish to make a public records request of your agency for the following records:
A list, log, assignment sheet or any other record that documents all t /re cellular and
land line telephone numbers used by Town employees in the course of their employment and
the employees to whom these numbers were assigned on 3 -5 -12.
All responses to this public records request should be made in writing,

If the public records being sought are maintained by your agency in an electronic format,
please produce the records in the original electronic format in which they
were created or received. See §119.01(2)(f), Florida Statutes.
If you contend that any of the records I am seeking, or any portion thereof, are
exempt from inspection or disclosure, please cite the specific exemption as required
by §199.07(1)(e) of the Florida Statutes and state in writing, and with particularity,
the basis for your conclusions as required by §119.07(1)(f) of the Florida Statutes.
Please take note of §119.07(c) Florida Statutes and your affirmative obligation to
(1) promptly acknowledge receipt of this public records request and
(2) make a good faith effort which "includes making reasonable efforts to determine
from other officers or employees within the agency whether such a record exists and,
if so, the location at which the record can be accessed" I am, therefore, requesting
that you notify every individual in possession of records that may be responsive
to this public records request to preserve all such records on an immediate basis.
If the requested records are not all readily available, I will accept them piecemeal.
I request that whichever records are readily available be provided to me immediately;
and those other records be provided to me as soon as possible.
If you anticipate the production of these public records to exceed $1.00, please
notify me in advance of their production with a written estimate of the total cost.
Please be sure to itemize any estimates so as to indicate the total number of pages
and/or records, as well as to distinguish the cost of labor and materials.

Subject: Telephone Records
Dear Rita:
This letter is a follow up to the information you provided in response to my previous public
records request dated June 18, 2013 (known as item "J "). We need to be able to match the
Town's land line and cell phone numbers with the employees associated with those numbers.
Therefore, on behalf of my client, Christopher O'Hare, and pursuant to the Public Records Act,
Chapter 119 of the Florida Statutes, l am herewith making a public records request for copies of
the following items:
1. A list, log, assignment sheet or am other record that docantents all the cellular and
land lime telephone nunthers used hl• Town entplgrees in the coarse of their
employment and the employees to whom these numbers were assigner! between 10 -27-
11 and 10- 29 -11; and
2. A list, log, assignment street or any other record that documents all the cellular and
land dine telephone numbers used by Town entplgrees in the course of their
employment and the employees to whore these numbers were assigned on 12- 28 -11;
and
3. A list, log, assignment sheet or am other record that documents all the cellular and
land line telephone numbers used by Town employees in the course of their
emplorntent and the employees to whom these numbers were assigned of 3 -5 -12; and
4. A list, log, assignment sheet or an other record that documents all the cellular and
land line telephone numbers used by Town entplorees in the course of their
employment and the employees to whom these numbers were assigned on 3- 20 -12; and
5. A list, log, assignment street or any other record that documents all the cellular and

and 9- 19 -12; and
6. A list, log, assignment sheet or any other record t hat documents all the cellular and
land line telephone munbers uses! by Town enuplol'ees in the course of their
enhplol'ment and the employees to whoa these numbers were assigned between 10 -1 -11
and 11 -5 -12; and
7. A list, log, assignment sheet or ant' other record that docwnents all the cellular and
land line telephone numbers used by Tovn employees in the course of their
ennploynnent and the employees to whom these n anthers were assigned between 2 -26 -13
and 5- 16 -13; and
8. In the alternative, a single list, log, assignment sheet or any other record that
documents all the cellular and land line telephone numbers used by Town employees
and the employees to hvhan these numbers are assigned ifsaid list, log, assignment
sheet or other record of said numbers and assigned enhpl {pees is the sane for all the
above requested time periods.
If you refuse to provide this information, Chapter 119 requires that you advise me in writing and
indicate the applicable exemption to the Public Records Act. Also, please state with particularity
the reasons for your decision, as required by Section 119.07(2)(a). if the exemption you are
claiming only applies to a portion of the records, please delete or redact that portion and provide
photocopies of the remainder of the records, according to Section 119.07(2)(a).
If you can provide an estimate of the cost of duplication, I would appreciate it. If acceptable, I
will pay the actual cost of duplication as defined in Section I I9.07(1)(a). However, if you
anticipate that in order to satisfy this request, "extensive use" of information technology
resources or extensive clerical or supervisory assistance as defined in Section I I9.07(1)(b) will
be required, please provide a written estimate and justification.
I would ask that you provide all the above requested records no later than this coming
MONDAY, JULY 18, 2013.
