Work Location: multiple devices for the Town
We hereby propose to furnish material and labor in accordance with the following
specifications at the referenced work location:
Option 1: price based on a (1 ) year contract. Price to test 100-170 backflows , price to test 2
inch and under $64.00 per device, add $25.00 per device for any over 2 inch backflows. Price
does not include any repairs for failed devices, proposal will be sent directly to Town of Gulf
Stream, if occupant uses others to repair and needs Buckeye to return and test they will be
responsible for new test to be performed.
Option 2: price based on a (3) year contract. Price to test 100-170 backflows , price to test 2
inch and under $70.00 per device, add $25.00 per device for any over 2 inch backflows. Price
does not include any repairs for failed devices, proposal will be sent directly to Town of Gulf
Stream, if occupant uses others to repair and needs Buckeye to return and test they will be
responsible for new test to be performed.
Exclusions: Work not included within this scope to be completed by others. Work that is outside the scope of our license
including but not limited to landscape removal/install/repair, sprinkler repair, electrical work, damage to lines below grade,
damage that may occur from saw -cutting of concrete, concrete cutting/patching, painting, drywall removaVrepair, pre-
existing damage &/or conditions. Any work that violates code or ordinance. Damage coming from or resultant of
deterior pre-existing conditions_ Work that is certain or likely to cause damage due to pre-existing conditions.

Conditions: Backfill to rough grade only. All material is guaranteed to be as specified and completed in a workmanlike
manner according to standard practices. Alteration and deviation from the specifications will result in additional costs to
the original contract. Changes to contract to comply with local code or changes required or imposed by municipalities,
HOAs or Condo Assoc. will be additional to Contract. There is no credit for furnished by owner materials or fixtures.
Materials/fixture furnished by owner/contractor will be subject to an additional installation charge and will void the warranty
provision provided by this contract.. Past due or delinquent amounts are subject to a finance charge up to the maximum
rate allowed by law_ Customers who do not pay within terms agree to reimburse BPI for !;&�
rredin collection.
This includes Notices, Liens, Legal, Attorney and Court costs. Venue Palm Beach County, (initial)
Cancel ations/Changes: Cancelation will be honored if the job has not been started, permit applications or materials
ordered. Deposits that have not incurred fees will be returned within 30 days. Cancelation or change of specialty or non -
stock items and materials will be subject to a restocking fee up to 40%. Cancellations made after down payment and
ordering of materials will be subject to a 25% return fee plus a potential restocking fee of up to 40% of the item/s cost. If
materials cannot be restocked o re rn we cannot provide credit. Customer agrees to pay costs incurred. Costs/Fees
incurred cannot be waived. (initial)
Cost of Work: As outlined above (1) year $64.00 each under 2inch (3) year $70.00 each under
2 inch
50% DEPOSIT UPON ACCEPTANCE / BALANCE IN FULL DUE UPON COMPLETION

Special Note: Cr Card payments are accepted and subject to a 3% processing charge in addition to

Expiration: This proposal will expire in 30 days.
Warranties: Materials are warranted per manufacturer specifications and may not include Labor. Please register
products with the Manufacturer for additional warranty or extended warranty purposes. Buckeye Plumbing provides a 30
day warranty outside of Manufacturer Warranty. There is no warranty on blockages.
Acceptance of Proposal: I am the authorized owner/representative of the property. I have
read and agree to the specifications, conditions and cost. I hereby accept the terms as
outlined. I authorize Buckeye Plumbing to perform the work. Payment will be made as
defined.
