Bruce and Kirk,
As discussed, it was felt that since there are no action items to put on the agenda for a June JFOC
Committee meeting, a meeting will not be scheduled. In lieu of a meeting this month and for
months in which we do not have a meeting, I am proposing that Joe and I prepare a short report
regarding plant and other business related operations so that you in turn can keep your
Board/Committee members updated. We also update the Fraser Sanitation District Board at our
monthly meeting.
My feeling is that, unless there is an urgent matter or something more significant that requires
action by the Committee, there would not be a need for a JFOC meeting until August or
September. However we will need to schedule a meeting in the fall to review and approve the
2007 Budget. If anyone feels that there is a need for a meeting, please contact me.
Enclosed is an Operations report that Joe has prepared with details of happenings at the treatment
plant. Also enclosed are test results and a copy of the State inspection Report. If you have any
questions, or would like additional details about any of the information in his report, please feel
free to call him directly.
At the February meeting we discussed the fact that John Walker would be writing to the State to
request an extension of time to complete the Consolidated Facility Well No. 2 and related
reports. An extension was approved for one year to March 10, 2007. In April, the pump
installation and test report etc. were completed and submitted to the State. The related
correspondence is enclosed.
As far as the accounting function, I can tell you that the checking account for the JFOC has been
set up and is operational. The account was opened in April and I have been using this account to
pay for operations and maintenance expenses. This account is at the Grand Mountain Bank in
Fraser. As previously discussed, funds of $152,931 that were being held in the Colotrust account
for the O &M Reserve have been transferred to this account. This amount is the equivalent of the
what each District has contributed toward the O&M Reserve fund for 2005 and 2006, plus
accrued interest.
I feel that it is appropriate to monitor the cash flow in this account for several months. After I
can get a feel for how the cash flow is working, I am thinking that at a portion of those funds
could be invested into a higher interest earning account at the Grand Mountain Bank which
would still allow easy access for emergency situations.
The Fraser Sanitation District auditor, Paul Backes (McMahan and Associates) has provided a
great deal of assistance in getting the books sorted out to assure that the JFOC Fund reflects all
of the expenses for 2006 back to January 1st. Included with the Fraser Sanitation audit for 2005,
Paul will be providing a separate page in the audit for the JFOC Fund and next year, the plan is
to do a separate audit for the JFOC Fund.
The books for the month of May are not closed yet as I am still waiting for bank statements, etc.
Once the accounting is completed, I will forward the reports to you. Once you have an
opportunity to review those reports and if there are any questions, or if there is specific
information that you are interested in and don't see it, you can let me know and we will fine tune
it.

SUPERINTENDENTS UPDATE
Plant Operations-
tia) 1. Have started removing covers from SBR, they are in bad shape might have to
replace all of them.
2. Have shut down #2 trains for repairs. Those include repacking return pump
also repair of recycle pump cord that was damaged by pump. Cord was to
long.
3. Centrifuge we shipped out for repairs in Feb, damaged in shipment back to us,
sent back to Andritz Mar 24 for more tile replacement. The scroll was
reinstalled in late Apr with at least one broken tile. We attempted to run
machine at the speeds that were factory set. High vibration alarms along with
shut down have been problems since. Andritz sent a service tech Jun 2 and
lowered primary bowl speed to 2800rpr. We are going to run with new
setting and see what happens.
4. UV system bulbs have over 12000 hrs of run time. Factory recommended
replacement is 12000 hrs. Lab results for fecal are still well below permit
levels. We will need to change some or all of the bulbs before winter. The
system has been running well since changing all the ballast boards to same
model number. NO NUANCE ALARMS YEA.
5. Ground water is up and the pump room is not flooding. We still have a few
small leaks, nothing like last year. Do not expect any more work from JTJ.
6. Landscape plan has been completed, with missing plants from last year also
installed. Only lost one aspen tree to the elk, not to bad.
7. Recently adopted ammonia criteria by State Health Dept. will have some type
of effect on the next permit. Most likely limits for ammonia will be higher
than now. Now they say you can dump more ammonia in cold -water streams
with out harm to receiving stream.
8. Plant performance has been great 98% removal of BOD, 96% removal of total
solids and ammonia of less than 1 mg/1. Infiltration was very high this year
about 2.2 MGD for peak hour. This caused removal percentages to drop.

Services Unit, Water Quality Control Division "the Division on the above referenced facility on
April 11, 2006. The assistance of Joe Fuqua was very helpful and greatly appreciated. I have
enclosed a copy of the inspection report for your records.

During the inspection, we observed that facility conditions are accurately represented in the permit.
Comments And Recommendations

part. The recommendations will enable your system to better conform to the requirements of
applicable design criteria or other industry standards:

The influent flume is equipped with an Endress Hauser Prosonic FMU 861 ultrasonic flow
level sensor that was last calibrated in January 2005. This device should be calibrated
annually.
The method of analysis should be referenced on the laboratory bench sheets.
Self-Reported Data Review
During the inspection self reported compliance data contained in your discharge monitoring reports
(DMR) were reviewed/discussed from February 2005 to December 2005 and the following violations
were discussed: two flow violations, one total mercury violation, the second WET test for 2005 was
not done. If you have been responding to our Compliance Assurance and Data Management

Dear Mr. McElhaney:
I apologize for not getting the enclosed information to you sooner. I have been tied up
with a few appellate briefs which, unfortunately, took longer than originally anticipated.
Anyway, I am enclosing the following for your review and comment:
1. Pump Installation and Test Report (Form GWS -32), prepared by Fraser Sanitation
District's Superintendent, Joe Fuqua;
2. Well Construction and Test Report (Form GWS -31), also prepared by Joe Fuqua;
3. A water supply system schematic prepared McLaughlin Water Engineers (now
ASCG), which shows the manhole well in question; and
4. Another schematic showing how the water supply enters the plant.
PLEASE NOTE, that I did not have Joe Fuqua sign the enclosed reports as he is not
authorized by law to either construct the well in question or to install the pump. If needed, I can
either have Joe or the District's president sign these reports.
It is my understanding that the other information you required is the name of the
contractor who constructed the well and installed the pump. The general contractor for plant
construction was John T. Jones Construction, whose address is 2213 7 Avenue North, PO Box
2424, Fargo, North Dakota 58108.
As I have summarized in previous correspondence, Well Permit No. 62350 -F is used in
the wastewater treatment processes associated with the District's new wastewater treatment
facility. The well is used in compliance with an approved augmentation plan (Case No. 5-
03CW133; a copy of which is enclosed). The well is basically a manhole which receives water
from the french drain system surrounding the facility and which is then pumped for facility
process uses. The well water is then immediately returned to the Fraser River.
I hope the information supplied herewith is sufficient for your purpose to finalize the well
permit. Please let me know if any additional information is needed. I will be happy to assist you
in any manner that I can.

Proof of well construction and pump installation must be submitted to this office prior to

reports affirming construction of the well and the installation of pumping equipment.
Well owners who wish to construct their own well and/or install pumping equipment
must first successfully complete an examination prescribed by the Board of Examiners of
Water Well Construction and Pump Installation Contractors as well as submit these
reports upon completion of the construction and/or installation.

Dear Mr. McElhaney:
This office represents Fraser Sanitation District. You may recall our telephone
conversation on February 22, 2006 regarding the referenced matter. The District is presently
putting together the information you requested. So that the District does not lose its well permit
status, I am hereby requesting an extension to file the necessary Well Construction and Pump
Installation Reports, as well as the other information you mentioned during our telephone
conference. I am enclosing a $200.00 check for this purpose. I am also enclosing a copy of the
correspondence received from the Division of Water Resources regarding the status of the well
permit, for your easy reference.
To summarize, the District has recently constructed a new wastewater treatment facility.
The well in question is used in the wastewater treatment processes. It was constructed pursizont
to an approved augmentation plan. The problem that we have encountered is that the well was
constructed as part of the plant construction process, and not "drilled" within the normal meaning
of that term. As it has been explained to me in the past, the well is basically a manhole which
receives water from the french drain system surrounding the facility and which is then pumped
for facility process uses. The well water is then returned to the Fraser River at the facility
effluent outlet.
Additional time is needed to provide the Division of Water Resources with the
information you have requested. The District is presently completing the Construction and Pump
Installation Reports and will also provide you with the name(s) of the contractor responsible for

construction and installation. Also, the District will provide you with as-built drawings.
Although this is not the normal well permitting process, I am hopeful this information will assist
j you with approving the well, as constructed. I anticipate that I can provide you with this
information within the next several weeks. 1
Please let me know if you would like a copy of the District's augmentation plan. For
your information, the District's augmentation plan was approved by Water Division No. 5 in
Case No. 03CW133.
At your earliest convenience, please extend the well permit to allow the District
additional time to complete the permitting process. Do not hesitate to call if there are any
questions.

NOTICE
This permit to construct a well was issued on 3/10/2005. The expiration date of the permit is 3/10/2006. In
order for the permit to remain valid, certain actions must be taken by the well owner. As of this date, the Well
Construction and Test Report has not been received. The Pump Installation and Test Report has not been
received. Furthermore, a request for extension of the expiration date have not been received by the Division of
Water Resources.
f7,1 This permit was approved under Section 37 -90- 137(2), Colorado Revised Statutes. The well must be constructed and the pump installed, prior to the expiration date of the permit. These reports are the
responsibility of the licensed water well contractors, or the well owner if the work was performed by the owner.
The Well Construction and Test Report (GWS -31) and the Pump Installation and Test Report (GWS -32) must
be received by the Division of Water Resources prior to the expiration date of the permit. These and other
forms are available on our website at http: /www. water .state.co.us /pubs /forms.asp.
If pumping equipment has not been installed in the well, the well owner may request, in writing, a one -time, one
year extension of the expiration date. The request must be received with a $200.00 (two hundred dollar) filing
fee prior to the expiration date and must provide a reason or reasons for not constructing the well, installing
pumping equipment and why the expiration date should be extended for an additional year.
If the well has been constructed, pumping equipment will not be installed by the expiration date and an
extension of the expiration date has not been approved, the permit will expire and be of no force or effect. It will
be necessary for you to obtain a new well permit by submitting a completed application for an existing well
along with a $480.00 (four hundred and eighty dollar) filing fee.
If the well will not be constructed, and you do not wish to pursue construction of a well, please advise us in
writing in order that we may formally expire the permit without requiring additional notification from you.
If the expiration date has already been extended for one year, the statute does not allow more than one
extension of time. If the well construction and the pump installation reports are not received by the
expiration date, the permit expires and is of no force or effect.
