Fereday moved to recommend TSI to City Council as contractor to Airport Master Plan. Anderson seconded and the motion carried unanimously. FUNDING FOR AIRPORT PROJECT: Chuck Sundby informed the Committee that the funding has finally come through and the Committee will be able to start construction in late July for our 1994 Airport Improvement Project. UTILITY WORK: ******** Tucker moved to request City Council to instruct City Crews to schedule utility work from Highway to new taxi way. Anderson seconded and the motion carried unanimously. Respectfully Submitted, Rick Fereda Secretary MINUTES MCCALL MUNICIPAL AIRPORT PRE -CONSTRUCTION CONFERENCE FAA/AIP NO. 3-16-0023-04 10:00 AM, JULY 20, 1994 A. GENERAL — INTRODUCTIONS — OPENING 1. Schedule and Scope of Work a. Scope is Schedules A, B, C, D and F. Revised contract amount is $1,210,345.13. Project funded 90% FAA, 5% State Aeronautics and 5% City of McCall. (Note Schedule E has been deleted). b. Final and Substantial Completion dates in Agreement were based on start of construction mid May. Completion times to be revised via C.O. #2. c. Time extensions — Contractor to justify. Engineer needs to approve. Handle by C.O. Delay in federal funding necessitates time extension. d. Liquidated damages are $500.00/calendar day. 2. Identification and Responsibilities of Contract Parties. a. Airport Sponsor/Owner — City of McCall. b. Engineers — Toothman-Orton Engineering Company and Howard Consultants, Inc. Toothman-Orton Engineering Company to provide construction observation and surveys. ii. Howard Consultants, Inc. to provide Engineer's Quality Assurance on Asphalt and Quality Control on subgrade preparation and aggregate placement. c. Prime Contractor — Seubert Excavators, Inc. i. Pipe, Drainage and Tie -downs: Stephenson Construction, Boise, Idaho ii. Pavement Milling: Benge Construction, Portland, Oregon iii. Slurry Seal, Paint Striping: Alert, Inc., Meridian, Idaho iv. Fencing: B & B Fencing, Boise, Idaho This list includes all subcontractors to date. 1 d. Contractor's Quality Control Lab — Intermountain Materials Testing, aggregate production, asphalt production and placement; Intermountain Materials Testing. 3. Contract Status a. The Owners have received Bonds and Insurance and signed Agreement with Contractor. b. FAA/State have approved Grant Contracts so funding is in place. c. Notice to Proceed is issued today, but will be effective July 25, 1994. d. Change Order #1 in process to delete Schedule E. Contractor executed? e. Change Order #2 will be prepared following more schedule review to revise Substantial and Final Completion dates. 4. Chain of Command a. Owners issue all communications to the Contractor via the Engineer. (Contractor to direct all communications through Engineer.) b. Contractor communicates with his sub -Contractors and is responsible for their actions and performance. c. Owner/Engineer communicate with FAA, Contractor is not to deal with FAA directly. Contractor will receive no direct response from the FAA. d. Owner will issue NOTAMs. Contractor to advise Engineer's office of Schedule and Changes. e. Toothman- Orton Personnel: Kevin Bissell, E.I.T., will be the full-time Project Representative and will direct activities of Toothman-Orton SubContractors. Chuck Sundby, P.E. and Tom Lemenager, P.E. of Toothman-Orton Staff will also be involved in administration and quality control capacities. f. City of McCall: Safety Officer, Fire Chief Dale Points, will monitor activities of all relative to Airport operational safety. Bill Keating will be in charge of City utilities and maintenance. 5. Coordination/Job Meetings a. Regularly Scheduled Project Meetings on Monday, 9:00 A.M. or ? Sponsor will hold meetings on Mondays at 9:00 A.M. b. Sponsor's Safety Officer to attend routinely. Fire Chief Dale Points can attend at the above -scheduled time. 2 c. Others as appropriate or affected parties, i.e. utility company representative or FBO's. If there is an issue of particular concern recommend notification of intent to discuss on preceding Friday. d. Engineer's Field Office shall be located in the Toothman-Orton Office in the McCall Air Taxi Building. Conference room will be used for weekly meetings. e. Contractor's Field Office ? . Seubert is preparing to locate a job trailer near the staging area. B. SAFETY AND OPERATIONS — SECTION 1000 & 1110 1 . Safety Officer and Notification of Parties a. Fire Chief Dale Points is Project Safety Officer for the Sponsor. Safety officer needs to be aware of all airport activities during construction. b. Contractor Safety Officer is Roger Hopt. c. USFS Operations Director is Merle Saleen. USFS operations are not to be interrupted. d. Public notification - suggestions? i.e., copies of Safety Plan at FBO's? 2. Temporary Lighting & Tiedowns a. Lighting on barricades. b. Work not expected to disrupt runway lights. /f work disrupts lighting, make provision for temporary lighting. c. Early activity - install temp tiedowns. d. Who moves aircraft to temp tiedowns? When? FBO will give one week notice and then will move aircraft. 3. Marking a. Temporary displaced threshold markings required. Detail in Plans. - Only required when proceeding with Schedule A. This item depends on the Contractor's schedule. b. Primarily cones and barricades to control ground movement. c. Be sure to maintain communication through Safety Officer. d. Date closures are to occur, issue NOTAMs. Send copies of NOTAM's to Sandy Simmons at FAA. 3 4. Security and Access a. Limited Access and travel routes; strictly enforced. No vehicles on the taxiway. b. Airport has ready access due to minimum fencing. Contractor must be aware of this and watch for casual observers. May need to add barricades or signs if a problem. Safety Officer also must be aware of this potential problem. Dale Points is considering additional warning signs. This was a problem on last project. The access road behind Pioneer Aviation needs to be oiled for dust control. T-O to pursue this with Contractor and implement. Seubert prefers access from Deinhard instead of Highway 55. Dale Points considers this acceptable. Intersection is dangerous, so avoid. Contractor's employees and equipment present a parking problem. Will use vacant areas behind hangars as staging area. Can leave equipment in apron infield area. 5. Hazard Identification a. Required at tie-in locations; construction activity locations. b. Construction markings at appropriate access points per Safety Plan or as stipulated by safety officer. c. Review at weekly meetings. Coordinate for upcoming weeks work. d. Lighted barricades and construction warning signs. May consider some type of general warning sign at entrance; none required now. Seubert is working on these items. 6. Damages a. Contractor is responsible for his and damages of subcontractors and the protection and restoration of property. b. Prompt repair is required. c. Contractor responsible for prompt repair of damages suffered as result of weather. Added work subject to C.O. request depending on circumstances. No guarantees. 7. Vehicular Traffic a. Limited to construction activity areas only and construction vehicles only. Designated workers access and parking area/yard area adjacent 4 to construction areas? Where? Parking is limited! Do not park work vehicles outside FBO's! Park behind hangars. b. Equipment marking while in aircraft operational areas, flags at minimum, flashers preferred on automobiles. c. Airplane Traffic and Operations have priority! d. No vehicle traffic on parallel taxiway or between the work areas by crossing diagonal taxiway or apron areas. e. Will need to monitor and coordinate with public vehicle access to hangar sites. This will involve the Airport Safety Officer and a warning sign(s). 8. NOTAMS a. Issued via Fire Chief Dale Points, Sponsor's Safety Officer, or ? b. Issue General NOTAM RE: Construction project - men and equipment working; effective next week. c. Advise Sandy @ FAA ASAP when issuing or changes to NOTAMS. 9. USFS Operations a. Anticipate a severe fire season - only time will tell. b. Safety Plan intended to minimize conflict with USFS operations in particular. c. If USFS operations become extensive, may necessitate project shutdown. 1989; a good example which would have required closure of 400 operations per day - mainly air tankers. d. USFS taxiway use by those who pass diagonal taxiway, i.e., jets, must be coordinated with UNICOM and USFS. Post UNIC0M directions. UNICOM operators should attend weekly construction meetings. This will occur during Construction Sequence #2. e. Additional comments - USFS? (Agreement.) An agreement with the USFS is necessary if their taxiway is used. Dale Points will coordinate with Merle Saleen 10. Additional Comments - Dale Points? Additional radio - hand held to monitor air traffic. Contractor's foreman. Bill Keating asked if the FBO parking lot will be cleared during construction. No, only contractor vehicles. 5 C. QUALITY CONTROL & TESTING - Reference Part 6, Section 2510. 1. Contractor's Independent Lab, Intermountain Materials Testing, is responsible for: a. Aggregate Materials Production Testing. Aggregate materials to include structural fill, subbase, base course and asphalt aggregates. b. Mix Design (50 Blow Marshall) and Quality Control Testing to include Asphalt Production and Placement in accordance with Mix Design parameters and construction specifications. Certified Lab ASTM D- 3666. IMT complies. IMT experienced quality control firm. T-O has worked with previously at McCall and elsewhere. c. Job Mix Formula must be Submitted & Approved by FAA prior to Placement! Allow 10 days minimum for review. — Review ASAP. Mix Design provided by Intermountain Mountain Testing. Be sure to submit in advance! d. Plan Administrator (Q.C) per P. 2510-21 to be Roger Hopt, Seubert Excavation. 2. Toothman-Orton Consultant responsible for: a. Subgrade Materials and Compaction Testing. b. Aggregate Materials Delivery and Placement Testing. c. Asphalt Acceptance and Payment Testing. d. Acceptance testing to be accomplished by an ASTM D-3666 Certified Lab. 3. Construction Management Plan, nearly complete, minimal additional information required. FAA approval needed. Note: Will complete when additional schedule information is provided. D. SURVEYING — SECTION 1050 1. Coordination a. Toothman-Orton to provide. b. Five working days notice required; crew may travel from Boise. Important to provide lead time so we can use local crew if possible. c. Kevin Bissell will be scheduling survey crew. Engineer will provide construction control staking (centerline, offsets, blue tops, etc.). Per Section 1050-3.1. Only one set of blue tops will be provided; be sure subgrade is very close before relinquishing blue tops. Roger Hopt is familiar with this potential problem. b. The Contractor shall provide a qualified, experienced individual whose primary responsibility is to monitor and supplement alignment and grade control staking throughout the project. Refer to P. 1050-3. c. Contractor responsible for transfer of horizontal and vertical control; confirm course elevations; auxiliary staking as needed for grade control. d. Restaking as a result of Contractor negligence or error, costs may be deducted. Engineer not responsible for time lost. Engineer may back charge for re -staking. 3. Record Drawings a. Contractor to maintain a working set of Drawings with drawings and notations to assist with the Engineers as constructed drawings — records. Contractor is to keep his set current and compare with the Engineer's set. T-O to prepare final as-builts. E. SUBMITTALS & SUBSTITUTIONS 1. Contractor Schedule and Safety Plan. (Should have in hand.) 2. Follow requirements of Section 1300. Submit early to allow review time. 3. Primary Equipment Submittals. Identify delivery schedule as best possible. 4. Minimal equipment or special materials. Not a big issue this project. Require a submittal on temporary tiedown cable. 7-O would like to see the tiedown submittal as soon as possible. F. CHANGE ORDERS, PAY REQUESTS 1. Follow chain of command with requests. Use a Field Order/Work Directive/ C.O. System. a. Field Order issued by Engineer based on pre -approved allowance. See appropriate form in the specs. b. Work Directive is approved by Owner, generally, minor or can become a Change Order. 7 c. Change Order or Supplemental Agreement is based on additional scope of work. FAA approval required on C.O.s. Normally can get telephone approval same day, if available. d. Identify additional costs or other problems up front — Do not come in with a list of extras at project completion. Bring up changes in a reasonable period of time. e. Documentation is paramount to approval of changes. 2. Force Account Allowance Bid Items a. Discuss procedure and rationale. Work with Field Order or Work Directive Form; i.e.; Item A22, $2,000.00. b. Provide invoice with man-hours, equipment, etc., to document costs. 3. Pay Request a. Contractor to submit to Engineer for review and recommendation. Engineer adds transmittal cover form in specifications to Contractor's request recommending approval. Would like to review Contractor's standard reporting form prior to first estimate. Submit example. Engineer requests Contractor's form in advance for review. b. Agreement allows up to 60 days to process. Have submitted Request for Reimbursement #1 so process is started. First usually takes longest. Contractor can submit request for this month to cover mobilization costs, if applicable. c. Desirable to have previous weeks quantities submitted by the Contractor and reviewed at each weekly project meeting. May avoid later conflict. d. Coordination of Submittal with City activities - City Manager to approve so rapid turnaround. When to submit? Submit to the City before the 2nd Monday of each month in order to schedule at first Council meeting. Council meetings are second and fourth Thursdays of the month. G. CIVIL RIGHTS AND LABOR REQUIREMENTS — SUPPLEMENTAL CONDITIONS 1. Subject to Davis -Bacon Rates per Contract. Submit payrolls weekly. SubContractors to submit to Seubert Excavators. Payrolls subject to audit. Toothman-Orton to review and submit to City. Discrepancies to be handled by the City with Contractor. Off -site employees and subcontractors are not subject to Davis -Bacon rates if operating a public pit which is not exclusively for use on project. 8 Contact: Marilyn Lerandeu, payroll clerk for Seubert Construction, Jim Henderson, City of McCall. 2. Veterans Preference Requirement included. 3. Subject to DBE Requirements of 8.5% for entire project. Seubert has identified compliance. Will need confirmation at end of Project. 4. Goals for minority and female participation are 3.0% minority and 6.9% female. 5. Engineer to conduct compliance interviews periodically. City will also do spot interviews. 6. Contractor responsible for sub -Contractor's actions in this area as well. include all labor provisions in subcontracts. H. EQUAL EMPLOYMENT OPPORTUNITY 1. EEO Poster publicly displayed. Contractor's work trailer if on -site. Posters not available; will provide prior to start of on -site construction. 2. Compliance inspection may be made by FAA. 3. Compliance report form to be submitted by Contractors. Contractor is familiar with and has previously complied with requirements. I. SPECIAL CONSTRUCTION TOPICS 1 . Contractor review of overall and Schedule sequence of operations. Contractor to describe: a. Discuss possible split work to next summer. May only do taxiway and pipeline this year. No apron and paving. See below. b. Schedule F Work - when? Separate budget. Late August. Stephenson Construction can't start until August 20, earliest. c. Work Hours? Contractor is unable to proceed with construction on July 25 as his employees and equipment are committed to other contracts. Delays in airport funding necessitated the prime Contractor and subcontractors to find other work while waiting for funds for this project. 9 Contractor's schedule to complete Sequence 1 would begin August 20 and end in mid -December. Since such a schedule is not acceptable, Contractor will prepare a summary of three or four alternative schedules which reflect subcontractor commitments as well as theirs, and present them to the Engineer. Engineer will assess and discuss with Owner. Contractor stated there may be added costs; i.e., change order requests associated with the delay in funding since may require equipment rental, remobilization, etc. Are trying to minimize this in scheduling. Want to be cooperative in this effort. T-O stated this consideration has already been discussed with FAA. 2. Work storage areas; ingress, egress and equipment routing. a. Employee parking and equipment storage? Behind hangars. 3. Dust Control. a. Contractor is responsible for supplying water for dust abatement. b. City hydrants not available for use! 4. Idaho Power and GTE utility line relocation. a. Desire to meet to discuss this issue separately. b. Contractor to trench, install conduit, backfill? (Not discussed.) c. Contractor to coordinate sequence with utilities. Utility relocation cannot be scheduled until Contractor presents a schedule. T-0 to notify utilities and meet to discuss sequence with them. 5. Permits and Licenses. b. Contractor to obtain all applicable permits and licenses. J. CLOSING - COMMENTS 1. Will prepare minutes and send copy to prime Contractor. Others desire a copy? 10
