ORDINANCE NO.
AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI, AUTHORIZING THE
MAYOR AND CITY CLERK TO EXECUTE AN AGREEMENT WITH CAPITAL REGION
MEDICAL CENTER FOR EMT RELATED SERVICES.
BE IT ENACTED BY THE COUNCIL OF THE CITY OF JEFFERSON, MISSOURI, AS
FOLLOWS:
Section 1. The Mayor and City Clerk are hereby authorized and directed to
execute an agreement with Capital Region Medical Center for EMT related services.
Section 2. The agreement shall be substantially the same in form and content
as the that agreement attached hereto as Exhibit A.
Section 3. This Ordinance shall be in full force and effect from and after the date
of its passage and approval.

AMENDMENT TO AGREEMENT FOR EMT SE R VICE,S
WHEREAS,the City of Jefferson,Missouri,a municipal corporation,hereinafter designated"City,"entered
into an Agreement with Capital liegion Medical Center, hereinafter referred to as "Medical Center," on
October 12, 2002 and amended on November 4, 2003;
WHEREAS, the Agreement was for a first responder service to be provided by City; and
WHEREAS,both parties wish to renew the Agreement fl0r the second cif Blur additional tenns ofone(1)year
and as stipulated in Paragraph 2 of'the Agreement dated October 12, 2002 a.nd amended on November 4,
2003; and
NOW,THEREFORE,be it agreed by the parties that the agreement is hereby renewed for the second of four
additional terms of one (1) year to end October 3 1, 2005.
IN TESTIMONY WHEREOF, the parties have hereunto set; their hands and seals this � 1

AMENDMENT TO AGREEMENT FOR EMT SERVICES
WHEREAS, the City of Jefferson, Missouri, a municipal corporation, hereinafter designated "City,"
entered into an Agreement with Capital Region Medical Center, hereinafter referred to as "Medical
Center," on October 12, 2002;
WHEREAS, the Agreement was for a first responder service to be provided by City; and
WHEREAS, both parties wish to renew the Agreement for a one (1) year period.
NOW, THEREFORE, be it agreed as follows:
A. Paragraph 2 of the Agreement dated October 12, 2002, is hereby amended to read as follows:
2. Duration and Termination.
This Agreement shall be in effect from November 1,209-22003 through October 31,
20% 2004. With the mutual agreement of both parties, the Agreement may be
extended on an annual basis for€aar-{4}three(3) additional terms of one (1) year
each. Either party shall inform the other, in writing, of its desire to renew or
terminate this Agreement for each one (1) year option with ninety J.90) notice days
before expiration of the Agreement. f-
B. All other sections of the Agreement dated October 12, 200N, shall remain in effect as stated.

WHEREAS, the Agreement was for a first responder service to be provided by City; and
WHEREAS,both parties wish to renew the Agreement for the third of four additional terms of one(1)year
and as stipulated in Paragraph 2 of the Agreement dated October 12, 2002 and amended on November 4,
2003, and December 21, 2004; and
NOW,THEREFORE,be it agreed by the parties that the_.agreemen_t is hereby renewed for the third of four
additional terms of one(1)year to end October 31, 2006.

WHEREAS, the Agreement was for a first responder service to be provided by City; and
WHEREAS,both parties wish to renew the Agreement for the forth of four additional terms of one(1)year
and as stipulated in Paragraph 2 of the Agreement dated October 12, 2002 and amended on November 4,
2003, December 21, 2004, and November 8, 2005; and
NOW,THEREFORE,be it agreed by the parties that the agreement is hereby renewed for the forth of four
— additional-terms-of one(1) year to end October 31, 2007.
