ORDINANCE NO. _ 12018
AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI, AMENDING THE
PROVISIONS OF THE CITY'S HEALTH PLAN PERTAINING TO MEDICAL BENEFITS,
EXCLUSIONS AND LIMITATIONS.
WHEREAS, The City has previously adopted the City of Jefferson Employee Health
Care Plan (hereinafter the "Health Plan"); and
WHEREAS, Certain amendments to the Health Plan are necessary and appropriate

NOW, THEREFORE, BE IT ENACTED BY THE COUNCIL OF THE CITY OF
JEFFERSON, MISSOURI, AS FOLLOWS:
Section 1. The current Health Plan is hereby amended by deleting the current
paragraphs "Dental Work" and "Orthognathic Surgery" and adding the following
provisions:
DENTAL WORK. No benefits will be paid for dental work treatment or x-ray,
charges of a physician, dentist, orthodontist, or dental surgeon (D.D.S.),
including removal of impacted wisdom teeth, except for supplies and
adjustments related to prosthesis devises to correct birth defects, or for
corrective treatment of an injury within six consecutive months following an injury
to a jaw or sound natural teeth, resulting from an accident sustained while
covered for this benefit.
ORTHOGNATHIC SURGERY. No benefits will be paid for any service or supply

received for corrections or deformities of the jaw. This consists of surgical
repositioning of portions of the upper or lower jaws or bodily restoration of entire
jaws. This exclusion will not apply to services or supplies related to surgery
needed to:
(1) correct birth defects; or
(2) correct a defect incurred due to accidental injury which, occurred while
covered under this plan.
