Any application required for any permit or license related to a home occupation shall include at least the following information: 1) the first and last '• ' name(s) of the persons operating the home occupation; 2) the specific nature of r the home occupation; 3) the address of the residence to be used for said home occupation; 4) whether or not the home occupation will involve the receiving of clients or customers, and if so, the anticipated volume and frequency of the '• same; and 5) the anticipated frequency of the deliveries related to the home occupation.
