ORDINANCE NO. 1/f:2
AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI, AUTHORIZING THE
MAYOR AND CITY CLERK TO EXECUTE AN AGREEMENT WITH FAMILY MENTAL
HEALTH CENTER FOR THE PROVISION OF MENTAL HEALTH SERVICES.
BE IT ENACTED BY THE COUNCIL OF THE CITY OF JEFFERSON, MISSOURI, AS
FOLLOWS:
Section 1. The Mayor and City Clerk are hereby authorized and
directed to execute an agreement with Family Mental Health Center
for the provision of mental health services. The amount of the
contract shall not exceed $5, 000. 00.
Section 2 . The agreement shall be substantially the same in
form and content as that agreement attached hereto as Exhibit A.
Section 3 . This Ordinance shall be in full force and effect
from and after the date of its passage and approval.

CONTRACT FOR PROFESSIONAL SERVICES
This Agreement, entered into this A8tt. day of 1991,
by and between the City of Jefferson, Miss uri, a Umunicipal
corporation, acting by and through its Mayor, hereinafter referred
` to as "City" and the Family Mental Health Center, not-for-profit
ti
organization existing under the laws of the State of Missouri,
hereinafter referred to as the "Center" .

WHEREAS, it is the desire of the City and the Center to
enhance the mental well-being of the citizens of Jefferson City, ;
NOW, THEREFORE, BE IT AGREED AS FOLLOWS : '
1. Effective January 1 1991 the Center willr�, .

J health services to the citizens of Jefferson City. For the
purpose of this agreement, mental health services will be �.

counseling, chothera

marital therapy, family therapy, group therapy,
chemotherapy, education and substance abuse counseling, and
crises intervention.

2. The Center will be able to provide consultation services to
it other public agencies in the City of Jefferson. Consultation
services will be defined as a process of providing case-
", oriented recommendations and/or treatment.
' 3. The primary objectives of the Center are to provide mental
` health services in a cost-effective and cost-efficient manner.
4. Mental health problems should be identified early and treated
`. within the least restrictive setting. As in all cases of
y_ health care, early identification prevents the development of
a more serious disease and precludes complications arising.
This philosophy supports working with the family, the school,
the business and the community.
5. Clients will be charged for mental health services according
to the following maximum rates:

and Crises Intervention $51. 76/hour
b. Psychotherapy/Counseling Services $50. 64/hour
C. Psychiatric/Medical Services $56.16/half hour
d. Psychological Testing Services $59. 24/hour

The actual amount a client will be expected to pay will be
based on a sliding scale. Fees will range from $5 . 00 per day E.
for a household of one with an annual income of $5, 600 to full
i fee with an annual income of $26, 000. Rates will decrease
with each additional family member.
6. Jefferson City employees and their immediate families residing
At the same address will. be offered mental health services at
one-half of the rates mentioned above. A full rate schedule
is attached to this contract as Appendix A. The Center will
advise Jefferson City employees and their immediate families
as to which charges may be reimbursable by the City' s health
insurance.
7 . Ways of handling employees with a problem, or problems, and
suggestions of how to refer employees to our agency for
professional assistance when needed will be 'available
throughout the year. Ongoing education/consultation services
for supervision or crises will be made available if. it is
requested by City Supervisors. ` 1

8. A sum of five thousand dollar ($5, 000. 00) will be granted to
Family Mental Health Center from the City of Jefferson for
providing mental health services for January 1, 1991, to
,k December 31, 1991, subject to the approval of the City
Council .
