CONTRACT FOR PROFESSIONAL SERVICES
This Agreement, entered into this 1_;T'�'day of 1992, by and between the
City of Jefferson, Missouri, a municipal corporation, nafter referred to as "City"

and the Family Mental Health Center, not-for-profit organization existing under the
laws of the State of Missouri, hereinafter referred to as the "Center".

WITNESSETH:
WHEREAS, it is the desire of the City and the Center to enhance the mental a
s: well-being of the citizens of Jefferson City,
#.'x NOW, THEREFORE, BE IT AGREED AS FOLLOWS:
1. Effective January 1, 1992, the Center will provide mental health services to �±
1 the citizens of Jefferson City. For the purpose of this agreement, mental
health services will be defined as a process of providing individual
psychotherapy-counseling, marital therapy, family therapy, group therapy,
chemotherapy, education and substance abuse counseling, and crises
intervention.
2. The Center will be able to provide consultation services to other public
agencies in the City of Jefferson. Consultation services will be defined as a
r' process of providing case-oriented recommendations and/or treatment.
,b 3. The primary objectives of the Center are to provide mental health services in
a cost-effective and cost-efficient manner.

4. Mental health problems should be identified early and treated within the least
restrictive setting. As in all cases of health care, early identification prevents
x the development of more serious disease and precludes complications arising.
This philosophy su pports working with the family, school, the business and
the community.
5. Clients will be charged for mental health services according to the following
maximum rates:
a. Evaluation/Assessment Services
and Crises Intervention $51.76/hour
' b. Psychotherapy/Counseling Services $50,64/hour
C. Psychiatric/Medical Services $56.16/half hour

.. The actual amount a client will be expected to pay will be based on a sliding

scale. Fees will range from $1.00 per day for a household of one with an
annual income of$5,600 to full fee with an annual income of$26,000. Rates
will decrease with each additional family member.
6. Jefferson City employees and their immediate families residing at the same
address will be offered mental health services at one-half of the rates
mentioned above. A full rate schedule is attached to this contract as Appendix
A. The Center will advise Jefferson City employees and their immediate
families as to which charges may be reimbursable by the City's health
insurance.

how to refer employees to our agency for professional assistance when needed

for supervision or crises will be made available if it is requested by City
Supervisors.

8. A sum of five thousand dollar ($5,000.00) will be granted to Family Mental
Health Center from the City of Jefferson for providing mental health services
for January 1, 1992, to December 31, 1992, subject to the approval of the City
Council.
