ORDINANCE NO.
AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI , AUTHORIZING AND DIRECTING
THE MAYOR AND CITY CLERK TO EXECUTE AN AGREEMENT WITH FAMILY MENTAL
HEALTH CENTER FOR PROVIDING MENTAL HEALTH SERVICES.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF JEFFERSON, MISSOURI,
AS FOLLOWS:
Section 1 . The Mayor and Clerk of the City are hereby authorized
and directed to execute a contract with Family Mental Health Center
for providing mental health services to the citizens of Jefferson City
and for providing free mental health services to employees of the City.
Section 2 . The contract shall be substantially the same in form
and content as the contract attached hereto as "Exhibit A" and made
a part of this Ordinance.
Section 3 . This Ordinance shall take effect and be in full force
from and after the date of its passage and approval. y

In an effort to enhance the mental well-being of the citizen:. of
Jefferson City, Family Mental Health Center and the-: City of Jefferson
affirm the following arrangement:
1. Effective November 1, 1963, Family Mental Health Center
will provide Mental Health Services to the citizens of J(-..ffers;on
City. For purpose of this agreement Mental Health Services shall
be defined as a process of providing itidiv.i.dual 1)sychntherat,v-
counseling, marital therapy, family therapy, group therapy, chemo-
therapy! education and crises intervention.
2. Family Mental Health Center will be available: to provide
consultation services to other public agencies in the City of
® Jefferson. Consultation service' s will be defined as a hr.oces!.; of
providing case-oriented recommendations and/or treatment.
.3. The primary objectives of Family Mental Health Center are
to provide Mental Health Services in a cost-effective and cost-
efficient manner.
4. Mental Health problems should be identified early and
treated within the least restrictive satti.ng. As in all eases of
health care, early identification prevents tho cievel.oinnc,nt of more
:serious disease and precludes complication:.; arising. Phis philo-
sophy supports working with the family, the school, the husine.::;s
and the community.

cording to the following maximum rates, subject to adjustment
changes in Fiscal Year 1983-84:
a. Evaluation/Assessment Service.,, $27.94/hr.
and Crises Intervention
b. Psychotherapy/Counseling Services $29.50/hr. .
c. Psychiatric/Medical Services $35.00/1/: hr.
The actual amount a client will be expected to pay will
be based on a sliding scale. Clients will be expected to pay a
monthly fee of approximately 109 of their monthly grass income,
excessive of the following amount:

6. Jefferson City employees and their immediate families re-
siding at the same address will be offered Mental Health Services
free of charge.
7. Ways of handling employees with a problem, or problems,
and suggestions of how to refer employea!3 to our agency for pro-
fessional assistance when needed will be available throuyhotit the
