SPONSORED BY COUNCILMAN ORDINANCE NO. f D 3 a AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI, AUTHORIZING THE MAYOR AND CITY CLERK TO EXECUTE A CONTRACT WITH THE DENTAL AND EYE PROGRAM FOR PROVIDING SERVICES PERTAINING TO EYE AND DENTAL SERVICES. BE IT ORDAINED BY THE COUNCIL OF THE CITY OF JEFFERSON, MISSOURI, AS FOLLOWS: Section 1. The Mayor and Clerk are hereby authorized and directed to execute a contract with the Dental and Eye Program for the provision of eye and dental services for the sum of $2, 100. Section 2. The contract shall be substantially the same in form and content as that contract attached hereto as Exhibit A. Section 3. This Ordinance shall be in full force and effect from and after the date of its passage and approval. Passed �� ev Approved iding fice � May ATTEST: City Clerk CONTRACT FOR SERVICES The Dental and Eye Program will provide free basic dental care, and free eye examinations and glasses to a minimum of 200 children of low income, Jefferson City families. This Program will be reimbursed $2, 100 by the City of Jefferson for the year 1985, to be paid semi-annually in equal installments. The funds for this program, including additional monies from United Way, the Jefferson .C:ity School District and the County of Cole, will be managed by the Board of Directors of the Dental and Eye Program, Marie Skain, Chairman ( 1422 Dixon Dr. , Jefferson City, MO) .
