ORDINANCE NO.
AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI, AUTHORIZING THE
MAYOR AND CITY CLERK TO EXECUTE AN AMENDMENT TO THE CONTRACT WITH
MEMORIAL COMMUNITY HOSPITAL FOR THE EMPLOYEE ASSISTANCE PROGRAM.
BE IT ENACTED BY THE COUNCIL OF THE CITY OF JEFFERSON, MISSOURI,
AS FOLLOWS:
Section 1. The Mayor and Clerk are hereby authorized and
directed to execute an amendment to the contract with Memorial
Community Hospital for the Employee Assistance Program.
Section 2 . The amendment to the contract shall be
substantially the same in form and content as that attached
hereto as Exhibit A.
Section 3. This Ordinance shall be in full force and effect
from and after the date of its passage and approval.

CONTRACT AMENDMENT
This contract amendment made this day of September,
1987, modifies the Agreement dated April 21, 1986, between
Memorial Community Hospital, a nonprofit corporation located at
1432 Southwest Boulevard, Jefferson City, Missouri, 65101 ,
hereinafter designated "Memorial, " and the City of Jefferson,
Missouri, a municipal corporation hereinafter designated "City. "
Memorial and City in consideration of their mutual covenants
herein agree in respect of the performance of services by
Memorial and the payment for those services by City as set out
below.
The agreement shall be modified as follows :
1. A new subparagraph (m) shall be added to paragraph ( 1 )
to read as follows :
(m) Provide physical examinations/health screens for
members of the Fire Department which shall include all
items in attachment "A" hereto. In addition to the
examinations/screens, Memorial will hold at least three
sessions of an educational/awareness building
presentation as well as an introduction to the physical
examination.
2 . Amend 2(e) by addition of an additional sentence to
read as follows : Any physical examination/health screen
provided under 1(m) shall be billed to the City at the
rate of $125 .00 per examination.
This modification shall be in full force and effect from
October 1, 1987, to December 31, 1987. The agreement shall
otherwise remain in full force and effect until its expiration
date.
