IQ 21?--r- AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI, AUTHORIZING THE MAYOR AND CITY CLERK TO EXECUTE AN AGREEMENT WITH FAMILY MENTAL HEALTH CENTER FOR THE PROVISION OF MENTAL HEALTH SERVICES. BE IT ENACTED BY THE COUNCIL OF THE CITY OF JEFFERSON, MISSOURI, AS FOLLOWS: Section 1. The Mayor and Clerk are hereby authorized and directed to execute an agreement with Family Mental Health Center for the provision of mental health services for a sum of $5,000.00 Section 2. The agreement shall be substantially the same in form and content as that contract attached hereto as Exhibit A. Section 3. This Ordinance shall be in full force and effect from and after the date of its passage and approval. Passed ��e /S, /��� Approved e �5�� Presiding ficer Mayor ATTEST: City Clerk AGREEMENT BETWEEN THE CITY OF JEFFERSON and FAMILY MENTAL HEALTH CENTER 1905 Stadium Blvd., Jefferson City, Missouri In an effort to enhance the mental well-being of the citizens of Jefferson City, Family Mental Health Center and the City of Jefferson affirm the following arrangements. 1. Effective November 1, 1987, Family Mental Health Center will provide Mental Health Services to the citizens of Jefferson City. For purpose of this agreement, Mental Health Services shall be defined as a process of providing individual psychotherapy-counseling, marital therapy, family therapy, group therapy, chemotherapy, education, substance abuse counseling, and crises intervention. 2. Family Mental Health Center will be available to provide consultation services to other public agencies in the City of Jefferson. Consultation services will be defined as a process of providing case-oriented recommendations and/or treatment. 3. The primary objectives of Family Mental Health Center are to provide Mental Health Services in a cost-effective and cost-efficient manner. 4. Mental Health problems should be identified early and treated within the lease restrictive setting. As in all cases of health care, early identification prevents the development of more serious disease and precludes complications arising. This philosophy supports working with the family, the school, the business and the community. 5. Clients will be charged for Mental Health Services according to the following maximum rates, subject to adjustment changes in Fiscal Year 1987-88: a. Evaluation/Assessment Services $30.14/hour and Crises Intervention b. Psychotherapy/Counseling Services 39.00/hour C. Psychiatric/Medical Services 46.26/half-hour d. Psychological Evaluation Services 43.36/hour 6. Jefferson City employees and their immediate families residing at the same address will be offered Mental Health Services at one half of the rates mentioned above, following three free counseling sessions provided under the Employee Assistance Plan with Memorial Community Hospital. Family Mental Health Center will advise Jefferson City employees and their immediate families as to which charges may be reimbursable by the City's health insurance. 7. Whys of handling employees with a problem, or problems, and suggestions of how to refer employees to our agency for professional assistance when needed will be available throughout the year. Ongoing education/consultation services for supervision, psychological evaluation services, or crises services will be made available if it is requested by City Supervisors. 8. A sum of five thousand dollars ($5,000) will be granted to Family Mental Health Center from the City of Jefferson for providing Mental Health Services from November 1, 1987 to October 31, 1988, subject to the approval of the City Council for Fiscal Year 1987-88.
