AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI, AUTHORIZING AND DIRECTING THE MAYOR AND CLERK OF THE CITY TO EXECUTE A WRITTEN CONTRACT WITH THE COLE COUNTY MENTAL HEALTH SERVICES. BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF JEFFERSON, MISSOURI, AS FOLLOWS: SECTION 1. The Mayor and Clerk of the City are hereby authorized and directed, for and in behalf of the City, to execute a written contract with the Cole County Mental Health Services. SECTION 2. Said contract shall read in words and figures as per the attached which is incorporated herein and made a part hereof by this reference. SECTION 3. This Ordinance shall take effect and be in force from and after its passage and approval. Passed Approved ;��Piit t�ZZA-, AW-,, esident of the CofinailO MAYOr PRO TAr4L,- ATTEST: City erk • AGREEMENT BETWEEN THE CITY OF JEFFERSON and COLE COUNTY MENTAL HEALTH SERVICES 1905 Stadium Drive, Jefferson City, Missouri In an effort to enhance the mental well-being of the citizens of Jefferson City, Cole County Mental Health Services and the City of Jefferson affirm the following arrangement: 1. Effective November 1, 1981 Cole County Mental Health Services will provide Mental Health Services to the citizens of Jefferson City, For purpose of this agreement Mental Health Services shall be defined as a process of providing individual psychotherapy-counseling, marital therapy, family therapy, group therapy, chemotherapy, education and crises intervention. 2. Cole County Mental Health Services will be available to provide consultation services to other public agencies in the City of Jefferson. Consultation services will be defined as a process of providing case-oriented recommendations and/or treatment. 3. The primary objectives of Cole County Mental Health Services are to provide Mental Health Services in a cost- effective and cost-efficient manner. 4. Mental Health problems should be identified early and treated within the least restrictive setting. As in all cases • of health care, early identification prevents the development of more serious disease and precludes complications arising. This philosophy supports working with the family, the school, the business and the community. Page Two 5. Clients will be charged for Mental Health Services according to the following maximum rates, subject to adjustment changes in Fiscal Year 1981-82: a.) Evaluation/Assessment Services and Crises Intervention $26.22/hr. b.) Psychotherapy/counseling Services $27.10/hr. c.) Psychiatric/Medical Services $30.56/l/2hr. The actual amount a client will be expected to pay will be based on a sliding scale. Clients will be expected to pay a monthly fee of approximately 10% of their monthly gross income, excessive of the following amount: Family Size Gross Monthly Income one person 526.00 two persons 688.00 three persons 850.00 four persons 1,012.00 five persons 1,174.00 six persons 1,336.00 each additional; add 30.00 6. Jefferson City employees and their immediate families residing at the same address will be offered Mental Health Services free of charge. 7. An educational seminar will be offered for supervisors to present skills and techniques on how to detect employees experiencing personal problems resulting in impairment of their work. Ways of handling employees with a problem and suggestions of how to refer them to our agency for professional assistance when needed will be provided. On-going education/consultation services for super- vision will be made available. J , � Page Three 8.
