jj FAMILY MENTAL HEALTH CENTER FOR PROVIDING MENTAL HEALTH SERVICES.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF JEFFERSON,'
I MISSOURI, AS FOLLOWS:
i' Section 1. The Mayor and Clerk of the City are hereby authorized',

and directed to execute a contract with Family Mental Health Center for;
providing mental health services to the citizens of Jefferson City and fort

II providing free mental health services to" employees of the City.
Section 2. The contract shall be substantially the same in form)
and content as the contract attached hereto as "Exhibit A" and made a
part of this ordinance. ,

,! Section 3. This ordinance shall take effect from and after its`
passage and approval.

AGREEMENT BETWEEN THE CITY OF JEFFERSON
and
FAMILY MENTAL HEALTH CENTER
® 1905 Stadium Boulevard, Jefferson City, Missouri
In an effort to enhance the mental well-being of the citizens of
Jefferson City, Family Mental Health. Center and the City of Jefferson
affirm the following arrangement:
1. Effective November 1, 1982, 'Family Mental.' Health Center
will provide Mental Health Services to the citizens of Jefferson
City. For purpose of this agreement Mental Health Services shall
be defined as a process of providing individual psychotherapy-
counseling, marital therapy, family therapy, group therapy, chemo-
therapy, education and crises intervention.
2. Family Mental Health Center will be available to provide
consultation services to other public agencies in the City of. -
Jefferson. Consultation services will- be defined as a process of
providing case-oriented recommendations and/or treatment.
3. The primary objectives of Family Mental Health Center are
to provide M&ntal Health Services in a cost-effective and cost-
efficient manner.
4. Mental Health problems should be identified early and
® treated within the least restrictive setting. As in all cases of
health care, early identification prevents the development of more
serious disease and precludes complications arising. This philo
sophy supports working with the family,' the school, the business
and the community.

® S. Clients will be charged for Mental Health Services ac- ,
cording to the following maximum rates, subject to adjustment
changes in Fiscal Year 1982--83:
a.) Evaluation/Assessment Services $27.96/hr.
and Crises Intervention
b.) Psychotherapy/Counseling Services $29.50/hr.

c.) Psychiatric/Medical Services . $35.00/l/2hr.
The actual amount a client will be expected to pay will
be based on a sliding scale. Clients wili be expected to pay a
monthly fee of approximately 10% of theix monthly gross income,
excessive of the following amount; ,

three persons 850.00
four persons 1,012.00
five persons . 1,174.00
six persons 1,336.00
each additional; add 30.00
6. Jefferson City employees and their 'immediate families re-
siding at the same address will be offered Mental Health Services
free of charge.
7. An educational seminar was offered for supervisors last
year to present skills and techniques on how to detect employees
experiencing personal problems resulting in impairment of their
work. Ways of handling employees with a problem and suggestions of
how to refer them to our agency for ,professional assistance when
needed were provided. On-going education/consultation services• for

supervision will be made available if it is requested by City
Supervisors.'
8. A sum of ten thousand dollars ($10,000.00) will be
granted to Family Mental Health Center from the City of Jefferson
for providing Mental Health Services from November 1, 1982, to
October 31, 1983, subject to the approval of the City Council for
Fiscal Year 1982-83.
