MISSOURI, AS FOLLOWS-:)
Section 1. That the Mai,ror and City "lerk of the City of
Jefferson be and they are hereby authorized, directed and empowered
to sign and execute for and in behalf of said City an application
for membership and for electric service with the Callaway Electric
600perative., which said application is as follows, to wit:
APPLICATION, FOR ly.EMBETiSHIP A,',1D FOR ELECTRIC SERVICE
The undersigned (hereinafter called the "Applicant") hereby
applies for membership in and agrees to purchase electric energy
from Callaway Electric Cooperative (herein called the "Cooperativell
upon the following tort.-is and conditions.
1. The A-ipplicant will Day to the Cooperative the sum of

2. The applicant will when electric energy becomes availabl

mises described below and will pay therfor mcnthly at rates to be

The applicant will pay a bill of at least $ 3.74 per month re-
gardless of the number of kilowatt hours tonsured.
3. The Applicant willcause his premises to be wired in
accordance with wiring s.)ecifications approved by the �'oopertaive.
IThe Applicant ' s house is approximately ##!::# feet from proposed
distribution line and ### feet from the nearest public- road.
4. The Applicant will cor ply with and'.e bound by the pro-
visions of the charter and bylaws of the Cooperative, and such
rules and regulations as may from time to time be adopted by the
Cooperative.
5. The Applicant, by paying a membership fee and becoi,ing
a member, assumes no personal liability or responsibility ponsibility, for any
debts or liabilities of the Cooperative., and it is expressly under
stoor that under the law his private property is xempt from exe-
ciition for any such debt or liability.
The acceptance of this application by the Cooperative shall
constitute an agreei-!-ent between the Applicant and the Cooperative,
abd the contract for electric service shr-11 continue in force for III
one -rear from tbedate service is made available by the Coo )crative
to the Applicant, and ti-;erafter until cancelled by at least 30 days
