AUTHORIZING NON-PAID LEAVE OF ABSENCE

WHEREAS, Rober~ Wadiak has requested an immediate six (6) month
non-paid leave of absence from his employment with the Borough; and
WHEREAS, the Governing Body has considered this request and desires
to approve the same subject to the following conditions:
1. Prior to Mr. Wadiak's return to employment with the
Borough, he shall supply a certification from a medical doctor that he is
capable of pedorming his job duties as a laborer with the Streets and
Roads Department; and
2. Mr. Wadiak successfully completes all of the pre-
employment testing required by the Borough upon his return to
work.
NOW, THEREFORE, BE IT RESOLVED by the Mayor and Council of the
Borough of Carteret, County of Middlesex, State of New Jersey that
Robert Wadiak is hereby granted a non-paid leave of absence for a period
of up to six (6) months subject to the above described conditions.
BE IT FURTHER RESOLVED that the Borough Clerk is hereby authorized
and directed to notify the payroll department of this action, to forward a
certified copy of this Resolution to Mr, Wadiak, and to place a copy of the
same in his personnel file.
Adopted this 31st day of January, 2002
and certified as a true copy of the
original on February 1, 2002.
