This Addendum supplements a certain Agreement originally entered into in 2001 and amended
the 1 Ith day of December, 2003 by and between the City of Richmond, Indiana, a municipal corporation
acting by and through its Board of Public Works and Safety (hereinafter referred to as the "City") and
Hylant Group of Carmel, Indiana (hereinafter referred to as the "Contractor").
WHEREAS, City entered into an Agreement with Contractor to provide health insurance consulting
services; and
WHEREAS, the Agreement expired on December 31, 2004; and
WHEREAS, the parties mutually desire to renew this Agreement.
NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, including
the above -stated recitals, the City of Richmond, Indiana, by and through its Board of Public Works and
Safety and Hylant Group of Carmel, Indiana hereby agree that the above -referenced Agreement entered
into between the City and Contractor shall be extended through and including December 31, 2005, and
that Contractor shall be paid an annual sum of Fifteen Thousand Dollars ($15,000.00) for said work. This
Agreement shall automatically renew for the subsequent year beginning on January 1, 2006 and extending
through and including December 31, 2006. Contractor shall be paid an annual sum of Twenty Thousand
Dollars ($20,000.00) for said work.
