Box 270 Jefferson City, Missouri 65102 1-573-526-2978 dbe@modot.mo.gov www.modot.mo.gov The City of St. Louis is pleased to notify you that your firm meets the requirements for certification as a Disadvantaged Business Enterprise (DBE) and/or an Airport Concession Disadvantaged Business Enterprise (ACDBE), in accordance with the Missouri Regional Certification Committee (MRCC) policies and procedures, and Title 49, Parts 23 and 26 of the Code of Federal Regulations. Attached is the MRCC joint certificate that reflects the effective date of your certification and the specific work types you are certified to perform. Work done by your firm on federally assisted projects in Missouri will count toward applicable DBE/ACDBE participation goals if your firm performs a commercially useful function, is responsible for a distinct element of the work of a contract, and carries out its responsibilities by actually performing, managing and supervising the work. Please note that your firm shall remain certified as a DBE/ACDBE until it withdraws from the DBE/ACDBE Program or is formally decertified by the MRCC. If there are any changes in the ownership, control, mailing address, e-mail address, telephone number or fax number of your firm, you must notify your certifying agency in writing immediately. In order to maintain your certification with the MRCC, you must submit the following to your certifying agency: •an annual update each year due by the firm’s anniversary date •a personal net worth statement due by each third year anniversary date It is your responsibility to ensure that your certification is kept up to date by submitting the required documentation as necessary. You may download the applicable forms from our website at http://www.mwdbe.org. Failure to submit annual updates, PNW statements and/or report any of the above changes to this office, or violation of the rules of the DBE Program may result in the revocation of your certification or other possible legal actions as set forth by Title 49 Part 26 of the Code of Regulations.
