THIS AGREEMENT made and entered into this _29th day of _December , 2006 and referred to
as Contract No. 100-2006 by and between the City of Richmond, Indiana, a municipal corporation acting
by and through its Board of Public Works and Safety (hereinafter referred to as the "City") and Hylant
Group of Carmel, Indiana (hereinafter referred to as the "Contractor").
WHEREAS, On May 4, 2001, City and Contractor entered into Contract 38-2001;
WHEREAS, Contract 38-2001 was an Agreement for Contractor to provide health insurance
consulting services;
WHEREAS, the Agreement was amended to reflect a renewal option in Contract No. 168-2003;
WHEREAS, in Contract 4-2005, the parties mutually desired to simultaneously amend and renew the
Agreement for calendar years 2005 and 2006; and
WHEREAS, the parties mutually desire to extend Contract 4-2005 until December 31, 2007 with the
City extending at a price equal to or less than that stated herein.
NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, including
the above -stated recitals, the City of Richmond, Indiana, by and through its Board of Public Works and
Safety and Hylant Group of Carmel, Indiana hereby agree that the above -referenced Agreement entered
into between the City and Contractor shall be extended through and including December 31, 2007, and
that Contractor shall be paid an annual sum of Twenty Thousand Dollars ($20,000.00) for complete and
satisfactory performance of the work required hereunder. This Agreement shall not be renewed or
extended beyond December 31, 2007.
