on that item.

This transmittal will serve as a letter form agreement to supplement the original agreement
and is subject to all of the conditions of the original agreement with the expanded scope of
work and modified fees outlined in this letter. Included are two copies of this letter form
agreement to amend our contract for the additional services. If you concur with these
changes please sign and return one copy of the agreement to us for our files. We will
consider the receipt of a Purchase Order as our notice to proceed with the additional work.
Please keep the other copy for your tiles.
