G) APPLICATIONS FOR BENEFITS - Officer Sean Kozak is transferring his total creditable service of 3 years, 7 months and 19 days from Washington Police Pension Fund to Plainfield Police Pension Fund in the amount of $23,191.62. He has chosen to pay $918.07 per pay period for 26 payrolls
beginning on the payroll dated August 1, 2014. Washington Police Pension Fund
has transferred to Plainfield Police Pension Fund $36,560.38. Officer Kozak is a Tier 1 member. H) UNFINISHED BUSINESS
I) NEW BUSINESS 1) Investment Performance Report from Sawyer Falduto Asset Management LLC.
